自拟补肾强骨汤联合经皮椎体后凸成形术治疗Kummell病疗效观察

摘　　要：目的:观察自拟补肾强骨汤联合经皮椎体后凸成形术(PKP)治疗kummell病(Ⅰ期/Ⅱ期)的临床疗效。方法:选取30例kummell病患者,随机分为2组,对照组(14例)采取PKP术治疗,观察组(16例)在PKP治疗的基础上口服补肾强骨汤治疗;评估2组患者术前及术后第2天、术后两周、术后四周的功能障碍指数(ODI)、疼痛视觉模拟量表(VAS)评分以及病椎的Cobb角的改变。结果:所有患者均顺利完成治疗。与术前相比较,术后第2天两组VAS评分及ODI指数均显著降低(P<0.05),伤椎Cobb角较术前均显著降低(P<0.05);在术后两周、四周,观察组VAS评分及ODI指数低于对照组(P<0.05),而2组伤椎Cobb角相比较,差异无统计学意义(P>0.05)。结论:自拟补肾强骨汤联合PKP治疗kummell病可有效缓解患者疼痛、改善功能,其疗效优于单纯使用PKP治疗。Objective:To observe the clinical efficacy of Bushen Qianggu decoction(补肾强骨汤)combined with percutaneous kyphoplasty(PKP)in the treatment of Kummell disease(stage I/II).Methods:30 patients with Kummell disease were randomly divided into two groups.The control group(14 cases)was treated with PKP,and the observation group(16 cases)was treated with Bushen Qianggu Decoction on the basis of PKP;The changes of Oswestry Disability Index(ODI),Visual Analogue Scale(VAS)and Cobb angle of injured vertebra were evaluated before and on the second day,two weeks and four weeks after operation.Results:all patients completed the treatment successfully.Compared with that before operation,the VAS and ODI of the control group and the observation group were significantly lower on the second day after operation(P<0.05),and the Cobb angle of the injured vertebra was significantly lower than that before operation(P<0.05);Two weeks and four weeks after operation,VAS and ODI in the observation group were lower than those in the control group(P<0.05),but there was no significant difference in Cobb angle between the two groups(P>0.05).Conclusion:the Bushen Qianggu decoction combined with PKP in the treatment of Kummell disease can effectively relieve pain and improve function,and its curative effect is better than that of PKP alone.
