宁心疏肝汤联合甲磺酸溴隐亭对高催乳素血症性不孕症患者kisspeptin、mi R-126、mi R-145表达的影响

摘　　要：目的探讨宁心疏肝汤联合甲磺酸溴隐亭对高催乳素血症性不孕症患者神经肽吻素(kisspeptin)、miR-126、miR-145表达的影响。方法纳入2018年3月至2021年2月在南京中医药大学盐城附属医院就诊的高催乳素血症性不孕症患者84例,采取前瞻性随机对照试验,以随机数字表法将患者分为观察组(42例)、对照组(42例)。对照组给予甲磺酸溴隐亭片治疗,观察组在对照组基础上给予宁心疏肝汤治疗。疗程均治疗3个月。比较两组临疗疗效,以及治疗前、治疗3个月后中医证候总积分及中医单项证候积分、性激素水平[血清催乳素(PRL)、黄体生成素(LH)、卵泡雌激素(FSH)、雌二醇(E2)],基础体温变化、kisspeptin、血清miR-126、miR-145表达水平、妊娠率及排卵率。结果观察组治疗后总有效率88.09%(37/42),高于对照组的71.43%(30/42)(P<0.05);两组治疗后中医症候总积分及单项中医证候积分、血清PRL水平、血清miR-126、miR-145表达水平均较治疗前降低,且观察组低于对照组(P<0.05);两组治疗后血清FSH、E2水平、血清kisspeptin表达水平均较治疗前升高,且观察组高于对照组(P<0.05);治疗后观察组妊娠率及排卵率高于对照组,观察组双相体温病例多于对照组(P<0.05),两组在改善血清LH水平方面差异无统计学意义(P>0.05)。结论宁心疏肝汤联合甲磺酸溴隐亭治疗可降低高催乳素血症性不孕症患者血清miR-145、miR-126表达水平,提高血清Kisspeptin表达,促进FSH、LH、E2释放,改善其中医症状,提高排卵率和妊娠率。Objective To investigate the effect of Ningxin Shugan Decoction combined with bromocriptine mesylate on the expression of kisspeptin,miR-126 and miR-145 in patients with hyperprolactinemia infertility. Methods A total of 84 patients with hyperprolactinemia infertility who were treated in our hospital from March 2018 to February2021 were enrolled. A prospective randomized controlled study was adopted,and the random number table was used to divide the patients into observation group(n=42)and control group(n=42). Patients in the control group were treated with bromocriptine mesylate tablets,and those in the observation group were treated with Ningxin Shugan Decoction on the basis of the control group. The course of treatment last for 3 months. The curative effects of the two groups were compared,as well as the total scores of TCM syndromes and the scores of individual TCM syndromes before and after 3 months of treatment. The sex hormone levels before and after treatment for 3 months included serum prolactin(PRL), luteinizing hormone(LH), follicular estrogen(FSH), Estradiol(E2), basal body temperature changes,neuropeptide kissin(kisspeptin),serum miR-126,miR-145 expression levels,pregnancy rate and ovulation rate were also compared. Results The total effective rate in the observation group was 88.09%after treatment,which was higher than 71.43% in the control group(P<0.05). After treatment,the TCM syndrome scores and individual TCM syndrome scores,serum PRL levels,expression levels of miR-126 and miR-145 of patients in two groups were lower than those before treatment,and which of the observation group were lower than the control group(P<0.05). The serum FSH,E2 levels,and the serum kisspeptin expression levels after treatment in the two groups were higher than those before treatment,and which of the observation group were higher than the control group(P<0.05).
