CT引导下经皮肺穿刺活检术对肺小结节诊断价值研究

摘　　要：目的探讨CT引导下经皮肺穿刺活检术对肺小结节诊断价值。方法选择2017年1月至2019年11月期间在本院行CT引导下经皮肺穿刺活检术的100例肺小结节患者作为研究对象,统计患者活检术后诊断结果,与患者开胸术后病理诊断结果对比,分类统计良性病变和恶性病变的诊断准确率。结果经皮肺穿刺活检术与术后病理诊断比较:良性病变诊断符合率为87.50%(21/24),肺癌诊断符合率为93.42%(71/76),二者比较,差异无统计学意义(P>0.05);良恶性病变诊断总符合率为92.00%(92/100),肺癌患者中,小细胞癌与术后病理诊断符合率最低,大细胞癌、肉瘤样癌符合率最高;良性病变中,肺真菌病与术后病理诊断符合率最低,硬化性血管瘤符合率最高。结论CT引导下经皮肺穿刺活检术可有效诊断肺小结节,准确性较高,临床指导意义强,但需注意做好并发症防控,提高检查安全性。Objective To investigate the diagnostic value of CT guided percutaneous lung biopsy for pulmonary nodules.Methods A total of 100 patients with pulmonary nodules who underwent CT guided percutaneous lung biopsy in our hospital from January 2017 to November 2019 were selected as research objects.The diagnostic results of patients after biopsy were statistically analyzed,and the diagnostic accuracy of benign lesions and malignant lesions were classified and statistically compared with the pathological diagnosis results of patients after thoracotomy.Results The diagnostic coincidence rate of benign lesions was 87.50%(21/24),and that of lung cancer was 93.42%(71/76).The overall diagnostic coincidence rate of benign and malignant lesions was 92.00%(92/100).Among the lung cancer patients,the coincidence rate of small-cell carcinoma and postoperative pathological diagnosis was the lowest,and that of large-cell carcinoma and sarcoma-like carcinoma was the highest.Among the benign lesions,the coincidence rate of pulmonary mycosis and postoperative pathological diagnosis was the lowest,and the coincidence rate of sclerosing hemangioma was the highest.Conclusion CT guided percutaneous lung biopsy can effectively diagnose pulmonary nodules with high accuracy and clinical significance.However,it is necessary to pay attention to the prevention and control of complications and improve the safety of examination.
