右美托咪定在下肢骨折患者麻醉中的应用效果及其对炎症因子的影响

摘　　要：目的观察右美托咪定在下肢骨折患者中的麻醉效果以及对炎症因子的影响。方法选取西安交通大学第二附属医院2021年1月-5月收治的96例下肢骨折患者,使用抽签法分为观察组和对照组,每组48例。观察组采用右美托咪定麻醉,对照组采用丙泊酚麻醉。比较两组炎症因子(IL-6、IL-10、TNF-α)水平、不同时刻的平均动脉压、心率、Ramsay评分。结果观察组麻醉后IL-6、IL-10、TNF-α水平低于麻醉前,且低于对照组,差异有统计学意义(P<0.05);两组在给药前平均动脉压与心率比较,差异无统计学意义(P>0.05);观察组给药10、30、60 min时患者的平均动脉压和心率指标均优于对照组,差异有统计学意义(P<0.05);观察组给药后Ramsay评分优于给药前,且给药10、30、60 min及手术结束Ramsay评分均优于对照组,差异有统计学意义(P<0.05)。结论右美托咪定在下肢骨折患者麻醉中应用能够明显改善患者炎症因子水平,较好的维持术中生命体征,获得良好的麻醉效果。Objective To observe the anesthetic effect of dexmedetomidine in patients with lower limb fractures and its influence on inflammatory factors.Methods A total of 96 patients with lower limb fractures admitted to the Second Affiliated Hospital of Xi’an Jiaotong University from January to May 2021 were selected and divided into observation group and control group by drawing lots,with 48 cases in each group.The observation group was anesthetized with dexmedetomidine,and the control group was anesthetized with propofol.The levels of inflammatory factors(IL-6,IL-10,TNF-α),mean arterial pressure at different times,heart rate,and Ramsay scores were compared between the two groups.Results The levels of IL-6,IL-10 and TNF-α in the observation group after anesthesia were lower than those before anesthesia,and lower than those in the control group,the difference was statistically significant(P<0.05).There was no significant difference in mean arterial pressure and heart rate between the two groups at pre-administration(P>0.05);while the mean arterial pressure and heart rate at 10,30,60,min after administration in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).The Ramsay score of the observation group after administration was better than that before administration,and the Ramsay score at 10,30,60 min after administration and at the end of operation was better than that of the control group,the difference was statistically significant(P<0.05).Conclusion The application of dexmedetomidine in anesthesia of patients with lower limb fractures can significantly improve the level of inflammatory factors in patients,maintain intraoperative vital signs and obtain good anesthesia effect.
