摘 要:目的观察山稔子油敷剂对缺血性压疮小鼠的治疗效果,并分析其潜在的机制。方法将60小鼠随机分成山稔子油敷剂组、模型组、安尔碘组,每组20只,均采用磁铁夹背法制备缺血性压疮模型,建模后分别在创面局部涂抹山稔子油敷剂、生理盐水与安尔碘Ⅱ型皮肤消毒剂,2次/天;另选择10只小鼠作为正常对照组,不进行特殊处理。观察建模的三组小鼠分组处理14 d的皮肤创面大体情况、创面面积及组织病理变化;分组处理7、14 d,采用ELISA法检测各组血清白细胞介素6(IL-6)、IL-10水平,Western blotting法检测各组皮肤组织血管内皮生长因子(VEGF)、Ⅰ型胶原蛋白(COL-Ⅰ)、Ⅲ型胶原蛋白(COL-Ⅲ)、碱性成纤维细胞生长因子(bFGF)、转化生长因子β(TGF-β)蛋白表达。结果模型组可见创面大面积肌纤维坏死溶解,肌原纤维排列疏松,周围可见较多的淋巴细胞以及单核细胞浸润;与模型组比较,山稔子油敷剂组及安尔碘组创面面积明显减小,肌纤维覆盖增加,淋巴细胞及单核细胞浸润减少,但山稔子油敷剂组变化更明显。分组处理7、14 d,模型组、安尔碘组、山稔子油敷剂组小鼠同一时间点创面面积依次降低,两组间比较P均<0.05。分组处理7、14 d,模型组、安尔碘组、山稔子油敷剂组、正常对照组小鼠同一时间点血清IL-6水平依次降低、IL-10水平依次升高,两组间比较P均<0.05。相同时间点,正常对照组小鼠皮肤创面组织VEGF、COL-Ⅲ、bFGF、TGF-β蛋白表达均高于模型组(P均<0.05)。分组处理7 d,山稔子油敷剂组皮肤创面组织VEGF、COL-Ⅰ、COL-Ⅲ、bFGF、TGF-β蛋白表达均高于模型组,安尔碘组皮肤创面组织VEGF、COL-Ⅰ蛋白表达均高于模型组(P均<0.05)。分组处理14 d,山稔子油敷剂组皮肤创面组织VEGF、COL-Ⅲ、bFGF、TGF-β蛋白表达均高于模型组,安尔碘组皮肤创面组织VEGF、COL-Ⅲ、TGF-β蛋白表达均高于模型组,山稔子Objective To observe the therapeutic effect of Fructus rhodomyrti oil dressing on ischemic pressure ulcer in mice,and to analyze its potential mechanism.Methods Sixty mice were randomly divided into three groups:Fructus rhodomyrti oil dressing group(n=20),model group(n=20),and Aner iodine group(n=20).The ischemic pressure ulcer was prepared by continuous magnet clipping on the back of mice.After modeling,we applied Fructus rhodomyrti oil dressing,normal saline and Aner iodine type Ⅱ skin disinfectant on the wound surface,respectively,twice a day;another 10 mice were selected as the normal control group without special treatment.The general condition,wound area and histopathological changes of the three groups of skin wounds were observed.ELISA was used to detect the levels of serum interleukin-6(IL-6)and IL-10 in each group at 7 and 14 days after treatment;Western blotting was used to detect the expression of vascular endothelial growth factor(VEGF),collagen Ⅰ(COL-Ⅰ),collagen Ⅲ(COL-Ⅲ),basic fibroblast growth factor(bFGF)and transforming growth factor β(TGF-β)proteins.Results In the model group,large-area muscle fiber necrosis and dissolution were observed on the wound surface,the myofibrils were loosely arranged,and many lymphocytes and monocytes were infiltrated around them.Compared with the model group,the wound area of the Fructus rhodomyrti oil dressing group and the Aner iodine group was significantly reduced,the coverage of muscle fibers increased,and the infiltration of lymphocytes and monocytes decreased,but the changes in the Fructus rhodomyrti oil dressing group were more obvious.After 7 and 14 days of treatment,the wound area of mice in the model group,Aner iodine group,and Fructus rhodomyrti oil dressing group decreased sequentially at the same time point,and there was statistical significance between every two groups(all P<0.05).After 7 and 14 days of treatment,the serum IL-6 levels of mice in the model group,Aner iodine group,Fructus rhodomyrti oil dressing group,and normal control group
