摘 要:目的 了解2019年陕西省手足口主要病原构成和流行规律,为制定陕西省手足口病防控策略提供依据。方法 收集2019年陕西省122家县(区)级疾病预防控制中心采集的手足口病例的血清分型结果和病例资料,分离肠道病毒核酸检测阳性样本,并对流行优势病原的肠道病毒VP1区进行PCR扩增和序列测定,应用最大邻接法(neighbor joining method)构建系统进化树,进而鉴定毒株的基因亚型。结果 2019年陕西省采集手足口样本4 687例,肠道病毒核酸阳性3 271例,阳性率69.79%。其中CV-A16型1 647例,占50.35%,为优势病原,其他肠道病毒占47.32%,EV-A71型占2.32%。从时间分布和地区分布看,陕西省各市辖区域内主要流行优势病原均为CV-A16,且EV-A71构成比均不足10%。2019年分离获得140株肠道病毒,其中68株为CV-A16型,经系统进化分析,32株均属于B1a基因亚型,36株属于B1b基因亚型。结论 陕西省2019年手足口病病原以CV-A16为流行优势病原,其基因亚型以B1a和B1b共存。Objective To understand the composition and prevalence of hand-foot-mouth disease(HFMD) in Shaanxi Province in 2019, and to provide a basis 122 counties(district) level centers for disease control and prevention inShaanxi for the prevention and control of HFMD inShaanxi Province. Methods The serotyping results and individual information of HFMD cases were collected from Province in 2019. At the same time, the individual patient information of samples was collected. The positive samples of enterovirus PCR test were isolated, PCR amplification and sequencing were performed on the VP1 region of the prevalent dominant pathogen, and a phylogenetic tree was constructed by NJ method to identify the gene subtypes of the enterovirus strains. Results In 2019, 4,687 hand-foot-mouth samples were collected in Shaanxi Province, of which 3 271 were positive for enterovirus, with a positive rate of 69.79%.Among them, 1 647 cases were CV-A16, accounting for 50.35%, which was the dominant pathogen, with other enterovirus accounting for 47.32%, and EV-A71 2.32%. In terms of time distribution and regional distribution, CV-A16 was the dominant pathogen in shaanxi province, and the proportion of EV-A71 was less than 10%. In 2019, 140 strains of enterovirus were isolated, among which 68 strains were CV-A16 type. According to phylogenetic analysis, 32 strains belonged to the B1 a gene subtype, and 36 strains belonged to the B1 b gene subtype. Conclusion CV-A16 was the predominant pathogen of HFMD in Shaanxi province in 2019, and the gene subtypes of CV-A16 coexisted with B1 a and B1 b.
