格林巴利综合征的中医分期论治经验

摘　　要：格林巴利综合征属中医“痿证”范畴,肺脾肾亏虚、渐及五脏是发病之本,热、湿、痰、瘀、毒为致病之标。马云枝教授临证根据疾病不同阶段分期论治,主张早期治疗应清补兼得,邪正兼顾,重在清肺中燥热之邪,多以清燥救肺汤或清燥汤为主方加减;中期顾护脾胃,标本兼治,常选用补中益气汤或参苓白术散为主方加减;晚期重在滋补肝肾,辨证施药,随症化裁,调护五脏,以肝肾亏虚为主者,方选独活寄生汤加减;阴阳两虚、元气衰败者,方选右归丸加减;气血两虚者,方选八珍汤加减;若兼夹瘀血之象,则选补阳还五汤加减。Guillain-Barre syndrome belongs to the category of ’wei syndrome’ in traditional Chinese medicine.Deficiency of the lung, spleen and kidney, and the five internal organs are the root of the disease. Heat, dampness, phlegm,blood stasis, and poison are the signs of disease. Professor MA Yun-zhi’s clinical treatment is based on different stages of the disease. She advocates that early treatment should be clear and tonic, and both evils and positives should be taken into account.The focus is on clearing the heat and heat in the lung. Most of them are Qingzao Jiufei Decoction or Qingzao Decoction as the main prescription in the mid-term care for the spleen and stomach, both symptoms and root causes are often used. Buzhong Yiqi Decoction or Shenling Baizhu Powder is often used as the main prescription;In the late stage, the focus is on nourishing the liver and kidney, applying medicine based on syndrome differentiation, and adjusting the five organs. For patients with liver and kidney deficiency, the prescription of Duhuo Jisheng Decoction is selected;For those with deficiency of both yin and yang and vitality, the prescription of Yougui Pill shall be selected;For those with deficiency of both qi and blood, Bazhen Decoction shall be selected;If combined with blood stasis, Buyang Huanwu Decoction plus or minus could be selected.
