基于《红楼梦》晴雯病案的疗效追踪浅析“虎狼之药”的应用

摘　　要：通过对《红楼梦》晴雯病案的原文描写进行疗效追踪来分析其真实治疗效果,并对其中存在争议的“虎狼之药”特别是麻黄、石膏的药性及临床使用进行分析思考,得出如下结论:(1)晴雯病案病机为外感内滞无争议,但原文中晴雯服用王太医的处方后疗效并不理想,笔者认为初期治法宜先祛邪后治里,合理使用麻黄等“虎狼之药”可能疗效更佳。(2)合理使用“虎狼之药”需要深入研究其药理、毒理、药物配伍,重视临床医师使用“虎狼之药”的个人经验。(3)“虎狼之药”在中医药治疗急危重症、传染病及疑难杂病有巨大优势,合理使用此类药物对中医急危重症传染病、疑难杂病诊治有重要意义。Through the original description of Qingwen’s case in A Dream of Red Mansions, the curative effect and the disease prognosis were tracked, so as to analyze the real therapeutic effect. The controversial ‘tiger and wolf medicine’,especially ephedra and gypsum, were analyzed and thought about their medicinal properties and clinical use, and the following conclusions were drawn:(1)There is no dispute that the pathogenesis of Qingwen’s case was external induction and internal hysteresis, but the curative effect of Qingwen was not ideal after taking the prescription of imperial physician WANG in the original text. The author thinks that the initial treatment should first eliminate pathogenic factors and then treat internal diseases,and rational use of ephedra and other ‘tiger and wolf medicine’ may have better curative effect.(2)The rational use of ‘tiger and wolf medicine’ requires in-depth study of its pharmacological toxicology and drug compatibility, and attention should be paid to the personal experience of clinicians in using ‘tiger and wolf medicine’.(3)‘Tiger and wolf medicine’ has great advantages in the treatment of acute and severe diseases, infectious diseases and complicated diseases in traditional Chinese medicine. Rational use of such drugs is of great significance to the diagnosis and treatment of acute and severe diseases in traditional Chinese medicine.
