摘 要:目的:评价同伴教育干预对高血压管理的效果。方法:检索中国生物医学文献数据库、维普数据库、万方数据库、中国知网、PubMed、Web of Science、the Cochrane Library,搜索同伴教育干预对高血压管理效果的随机对照试验(RCT),对符合纳入标准的文献采用RevMan 5.3软件进行Meta分析。检索时限为建库至2021年8月22日。结果:纳入9篇文献,涉及1 181例病人。Meta分析显示,同伴教育干预可改善病人收缩压[WMD=-10.15,95%CI(-10.94,-9.36),P<0.000 01]、舒张压[WMD=-4.60,95%CI(-6.42,-2.79),P<0.000 01]、总胆固醇[WMD=-1.20,95%CI(-2.31,-0.09),P=0.03]、自我管理能力[WMD=2.15,95%CI(1.59,2.70),P<0.000 01],但对血清三酰甘油改善不明显[WMD=-1.01,95%CI(-2.11,0.09),P=0.07]。亚组分析发现同伴教育干预可改善来自社区病人收缩压[WMD=-8.51,95%CI(-9.57,-7.45),P<0.000 01]及来自医院病人收缩压[WMD=-18.14,95%CI(-20.36,-15.93),P<0.000 01];可改善干预时间≥8个月病人的舒张压[WMD=-5.77,95%CI(-6.67,-4.87),P<0.000 01]。结论:现有研究证据表明,同伴教育干预对高血压管理有一定效果,但是受病人来源、干预时间影响,还需纳入更多高质量的研究分析。Objective:To evaluate the effect of peer education intervention on hypertension management.Methods:Randomized controlled trials(RCTs) on the effect of peer education intervention on hypertension management were searched from CBM,VIP,WanFang Database,CNKI,PubMed,Web of Science,and the Cochrane Library.Meta-analysis was performed on literature that met quality standards by using RevMan 5.3 software.The retrieval time was from inception to August 22,2021.Results:Nine articles were included,including 1 181 patients.Meta-analysis showed that peer education-based interventions could improve systolic blood pressure[WMD=-10.15,95%CI(-10.94,-9.36),P<0.000 01],diastolic blood pressure[WMD=-4.60,95%CI(-6.42,-2.79),P<0.000 01],total cholesterol[WMD=-1.20,95%CI(-2.31,-0.09),P=0.03],self-management ability[WMD=2.15,95%CI(1.59,2.70),P<0.000 01].However,there was no significant improvement in serum triacylglycerol[WMD=-1.01,95%CI(-2.11,0.09),P=0.07].Subgroup analysis showed that peer education intervention could improve the systolic blood pressure of patients from community[WMD=-8.51,95%CI(-9.57,-7.45),P<0.000 01],patients from hospital[WMD=-18.14,95%CI(-20.36,-15.93),P<0.000 01],and diastolic blood pressure in patients with intervention time ≥ 8 months[WMD=-5.77,95%CI(-6.67,-4.87),P<0.000 01].Conclusion:Current evidence shows that peer education intervention has a certain effect on hypertension management,but due to the source of patients and the intervention time,more high-quality researches and analysis need to be included.
