清肺化痰针刺法联合桑白皮汤加味治疗重症肺炎(痰热壅肺证)的疗效研究

摘　　要：目的观察清肺化痰针刺法联合桑白皮汤加味治疗重症肺炎(痰热壅肺证)的疗效。方法选择88例患者按照随机数字表法分为对照组与治疗组各44例。对照组给予西医常规治疗,治疗组加用清肺化痰针刺法联合桑白皮汤加味治疗。连续治疗10 d,比较两组急性生理和慢性健康状况评分Ⅱ(APACHEⅡ)、临床肺部感染评分(CPIS)、痰热壅肺证症状评分、临床疗效以及血清C反应蛋白(CRP)和降钙素原(PCT)水平。结果治疗后,两组APACHEⅡ、CPIS、痰热壅肺证症状评分显著降低,且治疗组显著低于对照组(P<0.01)。治疗组的总有效率为95.45%,明显高于对照组的77.27%(P<0.05)。治疗后,两组血清CRP和PCT水平明显降低,且观察组明显低于对照组(P<0.01)。结论在西医常规治疗基础上,清肺化痰针刺法联合桑白皮汤加味治疗重症肺炎(痰热壅肺证),有助于患者症状体征和中医证候的改善,提高临床疗效,抑制炎症反应。Objective:To investigate the efficacy of Qingfei Huatan acupuncture method combined with Sangbaipi Decoction on severe pneumonia with syndrome of phlegm-heat obstructing lung.Methods:A total of 88 patients were selected and divided into the control group and treatment group according to random number table method,44 cases in each group.The control group was given conventional treatment measures of western medicine,and the treatment group was added with Qingfei Huatan acupuncture method combined with Sangbaipi Decoction.After 10 days of continuous treatment,scores of acute physiological and chronic health status scoreⅡ(APACHEⅡ),clinical pulmonary infection score(CPIS),scores of phlegm-heat obturation syndrome symptom,the clinical efficacy,serum levels of C-reactive protein(CRP)and procalcitonin(PCT)were compared between the two groups.Results:After treatment,the symptom scores of APACHEⅡ,CPIS,and scores of symptoms of syndrome of phlegm-heat obstructing lung in the two groups were significantly decreased,and the treatment group was significantly lower than the control group(P<0.01).The total effective rate of treatment group was significantly higher than that of control group(P<0.05).After treatment,the serum levels of CRP and PCT in the two groups were significantly decreased,and the observation group was significantly lower than the control group(P<0.01).Conclusion:Based on conventional western medicine treatment,the efficacy of Qingfei Huatan acupuncture method combined with Sangbaipi Decoction on severe pneumonia with syndrome of phlegm-heat obstructing lung is conducive to improving the symptoms and signs of patients and TCM syndrome scores,increase the clinical efficacy,and inhibit inflammatory reaction.
