摘  要:目的:分析南充市育龄期妇女的TORCH筛查结果,了解该地区育龄期妇女TORCH的感染现状及流行特点,为优生优育工作提供数据支撑。方法:选取2017年1月1日—2019年12月31日在川北医学院附属医院进行TORCH筛查的育龄期妇女3654人,采用化学发光免疫分析法检测其血清TORCH IgM和IgG抗体,包括弓形虫(TOX)、风疹病毒(RV)、巨细胞病毒(CMV)、单纯疱疹病毒(HSV)的IgM、IgG抗体共8项。结果:TOX-IgM、RV-IgM、CMV-IgM和HSV I/II-IgM阳性率分别为0.82%、2.93%、0.33%和20.66%;TOX-IgG、RV-IgG、CMV-IgG和HSV I/II-IgG阳性率分别为4.82%、81.09%、96.50%和83.17%。该地区及其他11个地区血清TORCH抗体检测结果显示,TORCH抗体阳性率存在地域差异,南充市HSV I/II-IgM阳性率最高。RV-IgG、HSV I/II-IgM、HSV I/II-IgG和CMV-IgG阳性率在低龄组和高龄组间比较,差异有统计学意义(χ^(2)值分别为44.966、5.136、25.338、8.075,P值均<0.01)。TOX-IgG、RV-IgM和HSV I/II-IgM阳性率在不同季节间比较,差异有统计学意义(χ^(2)值分别为9.096、11.464、26.117,P值均<0.01)。RV-IgM和HSV I/II-IgM阳性率在不同年份间比较,差异有统计学意义(χ^(2)值分别为32.058、118.389,P值均<0.01)。结论:HSV I/II在南充市育龄期妇女中有较高的活动感染率,CMV、HSV I/II和RV有较高的既往感染率。TORCH感染存在地域差异、季节差异和年龄差异,因此,各地区孕龄期妇女TORCH的血清学筛查结果分析对TORCH感染预防策略的制定具有重要意义。OBJECTIVE To analyze the results of TORCH screening and investigate the infection status and epidemic character⁃istics of TORCH among women of childbearing age in Nantong,so as to provide supporting data for prenatal and postnatal care.METHODS 3654 women of childbearing age who underwent TORCH screening from January 1,2017 to December 31,2019 in Affil⁃iated Hospital of North Sichuan Medical College were selected.Serum TORCH IgM and IgG antibodies were detected by chemilumi⁃nescence immunoassay.RESULTS The positive rates of TOX-IgM,RV-IgM,CMV-IgM and HSV I/II-IgM were 0.82%,2.93%,0.33%and 20.66%.Meanwhile,the positive rates of TOX-IgG,RV-IgG,CMV-IgG and HSV I/II-IgG were 4.82%,81.09%,96.50%and 83.17%.Analysis and comparison of serum TORCH antibodies in this region and 11 other regions showed that there were regional differences in TORCH antibodies,and the positive rate of HSV I/II-IgM in Nantong was the highest.The detection rates of RV-IgG,HSV I/II-IgM,HSV I/II-IgG and CMV-IgG were compared between younger and older groups and were statisti⁃cally significant(χ^(2)=44.966,5.136,25.338,8.075,respectively,P<0.01).The detection rates of TOX-IgG,RV-IgM and HSV I/II-IgM were compared between seasons and statistically significant(χ^(2)=9.096,11.464,26.117,respectively,P<0.01).The RV-IgM and HSV I/II-IgM detection rates were compared between years with significant differences(χ^(2)=32.058,118.389,respec⁃tively,P<0.01).CONCLUSION HSV I/II in women of childbearing age had a higher active infection rate in Nantong,while CMV,HSV I/II and RV had a higher previous infection rate.In view of regional differences,seasonal differences and age differences in TORCH infection,analysis of the results of TORCH screening in women of childbearing age was of great significance for the formula⁃tion of prevention strategies for TORCH infection.
