眼眶的三维CT形态指标在法医学年龄推断中的应用价值

摘  要:目的评价眼眶的三维CT影像形态指标在法医学年龄推断中的应用价值,为法医学年龄推断提供依据。方法收集322例8~20岁华北地区汉族人群眼眶三维CT影像,测量双侧眶底水平深度和垂直深度及眼眶前、中、后冠状面上的眶底角(眶底角1~3)大小。组间各指标比较采用单因素方差分析。以样本数据建立推断个体是否年满12、14、16及18岁的Logistic回归模型,绘制受试者工作特征曲线(ROC)评估各模型推断年龄的效能。结果男性人群中,各个年龄组间5个指标比较差异均有显著性(F=3.90~19.26,P<0.05);女性人群中,各个年龄组间眶底水平深度、眶底垂直深度、眶底角3三个指标比较差异有显著性(F=4.66~14.42,P<0.05);各个组别间眶底角1、眶底角2比较,差异无显著性(P>0.05)。推断个体是否年满12、14、16及18岁的ROC曲线下面积(AUC):男性分别为0.89、0.84、0.78、0.74;女性分别为0.85、0.83、0.66、0.65。结论眼眶三维CT影像形态指标适用于个体12、14岁年龄节点的推断。Objective To evaluate the application value of three-dimensional CT morphometric indices of the orbit in forensic age estimation, and to provide a basis for age estimation in forensic medicine. Methods The three-dimensional orbit CT images of 322 individuals of Han ethnic group aged 8-20 years living in North China were collected to measure the horizontal and vertical depths of bilateral orbital floor and the orbital floor angles(1-3) on the anterior, middle, and posterior coronal planes. All indices were compared between groups by one-way ANOVA. Logistic regression models were established based on sample data to estimate the age of subjects(12, 14, 16, and 18 years). Receiver operating characteristic(ROC) curves were plotted to evaluate the effectiveness of each model in age estimation. Results There were significant differences in five indices among different age groups of male subjects(F=3.90-19.26,P<0.05). There were significant differences in horizontal orbital floor depth, vertical orbital floor depth, and orbital floor angle 3(F=4.66-14.42,P<0.05) but no significant differences in orbital floor angles 1 and 2(P>0.05) between different age groups of female subjects. The areas under the ROC curves for estimation of 12, 14, 16, and 18 years of age were 0.89, 0.84, 0.78, and 0.74, respectively, for males and 0.85, 0.83, 0.66, and 0.65, respectively, for females. Conclusion The three-dimensional CT morphometric indices of the orbit are suitable for the prediction of ages of 12 and 14 years.
