摘  要:目的:基于医院收治的糖尿病住院患者医保相关数据,分析DIP支付方式实施前后对于患者住院费用造成的影响,评估DIP支付方式实施效果。方法:选取广州市某三甲医院2016年6月1日至2020年12月31日参加基本医疗保险的糖尿病住院患者为研究对象,通过广义线性模型,分析实施DIP对患者例均总住院费用、例均自付费用、例均自付费用占比和平均住院日的影响。结果:DIP的实施使糖尿病住院患者例均总住院费用下降6.5%,例均自付费用下降11.1%,例均自付费用占比下降1.3%。结论:DIP的实施可有效控制糖尿病患者例均总住院费用的增长,减少西药费、化验费和材料费等成本,有利于减轻患者个人住院费用负担,具有推广意义。Objective:Based on the medical insurance data of diabetes inpatients,the paper analyzes the impact of DIP on patients’ hospitalization expenses.Methods:We chose diabetes hospitalized patients who participated in basic medical insurance from June 1,2016 to December 31,2020 in a Grade Ⅲ Level A hospital in Guangzhou.The generalized linear model was used to analyze the effect of DIP payment method on the average total hospitalization expenses,the proportion of average self-paid expenses and the average length of stay of patients.Results:The implementation of the DIP payment system reduces the average total hospitalization expenses of diabetic inpatients by 6.5%,average self-paid expenses by 11.1%,proportion of average self-paid expenses by 1.3%.Conclusion:The implementation of the DIP payment system can effectively control the growth of average total hospitalization expenses of diabetes hospitalized patients,reduce the cost of western medicine,laboratory and materials,and at the same time,reduce the burden of individual hospitalization expenses.The DIP payment system has the practical significance of promotion.
