血清降钙素和癌胚抗原与甲状腺结节的鉴别诊断研究

摘　　要：目的探讨血清降钙素(PCT)和癌胚抗原(CEA)在鉴别诊断甲状腺结节中的作用。方法分别检测112例甲状腺髓样癌(MTC)患者(MTC组)、560例乳头状甲状腺癌(PTC)患者(PTC组)及260例良性结节者(良性结节组)的血清CEA和PCT水平并进行比较,计算单个指标的敏感性及特异性。运用受试者工作特征(ROC)曲线分别检测甲状腺髓样癌-乳头状甲状腺癌(MTC-PTC)组、MTC-良性结节组和PTC-良性结节组曲线下面积,并分析CEA和PCT对甲状腺结节的诊断价值。结果MTC组患者的PCT和CEA值均有明显升高,与其他两组比较且差异均有统计学意义(P<0.05)。血清PCT检测的敏感性和特异性分别为96.91%和99.01%,血清CEA检测的敏感性和特异性分别为79.44%和92.45%。PCT和CEA在PTC组和良性结节组之间的差异无统计学意义(P>0.05)。结论血清PCT和CEA的检测有助于甲状腺结节的鉴别诊断。Objective To discuss the values of serum procalcitonin(PCT)and carcinoembryonic antigen(CEA)in diagnosis of thyroid nodules.Methods The levels of the serum PCT and CEA in 112 patients with medullary thyroid carcinoma(MTC),560 patients with papillary thyroid carcinoma(PTC),and 260 patients with benign thyroid nodules were measured and compared,and the sensitivity and specificity were of each index were determined.ROC curve was used to detect the area under the curve of the medullary thyroid carcinoma-papillary thyroid carcinoma(MTC-PTC)group,the MTC-benign nodule group and the PTC-benign nodule group,respectively,and the diagnostic value of CEA and PCT in thyroid nodules was analyzed.Results Compared with the other two groups,both the serum PCT and CEA levels of patients who had MTC group were significantly higher(P<0.05).The sensitivity and specificity of the serum PCT were 96.91% and 99.01%.The sensitivity and specificity of the serum CEA were 79.44% and 92.45%.But in the levels of both PCT and CEA,there were no statistically significant differences between the PTC group and the benign thyroid nodules group(P>0.05).Conclusion The detection of serum PCT and CEA can help in the differentiation and diagnosis in thyroid nodules.
