关节镜手术联合腔内药物注射治疗半月板损伤的疗效及对疼痛因子、HSS量表、IKDC评分的影响

摘　　要：目的探讨关节镜手术联合腔内药物注射治疗半月板损伤的疗效及对疼痛因子、膝关节疼痛(HSS)量表、膝关节功能(IKDC)评分的影响。方法方便选择2019年5月—2021年5月在该院就诊的120例半月板损伤患者,采用随机数表法分组,关节镜手术治疗患者(n=60)纳入对照组,关节镜手术、腔内药物注射联合治疗患者(n=60)纳入研究组,对比两组的治疗结局。结果研究组治疗后HSS评分(19.02±1.21)分、lKDC评分(77.35±8.12)分以及膝关节活动度(132.25±13.33)°,优于对照组,差异有统计学意义(P<0.05);研究组治疗后疼痛因子水平中,5-羟色胺(5-HT)(0.19±0.02)μmol/mL、去肾上腺素(NE)(1.32±0.13)pg/mL、P物质(SP)(56.82±5.71)ng/mL,优于对照组,差异有统计学意义(P<0.05);研究组治疗有效率100.00%高于对照组的83.33%,差异有统计学意义(χ^(2)=10.909,P<0.05);两组治疗后的并发症发生率差异无统计学意义(P>0.05)。结论半月板损伤患者接受关节镜、腔内药物注射联合治疗,可以有效缓解患者的膝关节疼痛程度,加强患者膝关节功能以及膝关节活动度,提高患者治疗效果的同时,具有较高的安全性和显著的临床意义。Objective To investigate the efficacy of arthroscopic surgery combined with intracavitary drug injection in the treatment of meniscus injury and its effect on pain factors,knee pain(HSS)scale and knee function(IKDC)score.Methods 120 patients with meniscus injury treated in the hospital from May 2019 to May 2021 were conveniently selected and divided into two groups by random number method.Patients treated with arthroscopic surgery(n=60)were selected as the control group,and patients treated with combined arthroscopic surgery and intraventricular drug injection(n=60)were selected as the study group,and the treatment outcomes of the two groups were compared.Results The HSS score(19.02±1.21)points,the lKDC score(77.35±8.12)points,and the degree of motion of the knee joint(132.25±13.33)°in the study group after treatment were better than those of the control group,the difference was statistically significant(P<0.05).The pain factor levels of the study group after treatment were 5-HT(0.19±0.02)μmol/mL,NE(1.32±0.13)pg/mL,and SP(56.82±5.71)ng/mL,which were better than those of the control group,the difference was statistically significant(P<0.05).The effective rate after treatment in the study group was 100.00% higher than that in the control group's 83.33%(χ^(2)=10.909,P<0.05);there was no statistically significant difference in the incidence of complications between the two groups after treatment(P>0.05).Conclusion For patients with meniscus injury,the combined treatment of arthroscopy and intracavitary drug injection can effectively relieve the knee joint pain of the patient,and strengthen the knee joint function and knee mobility of the patient.While improving the treatment effect of patients,it has high safety and significant clinical significance.
