摘　　要：目的:在我国有自杀史的精神科住院病人中,探索自杀意念是否存在亚型。方法:采用方便取样对天津市某精神病院27名有自杀史的住院病人进行自然住院周期内(1-28天)每日随机4次的自杀意念生态瞬时评估(实时纸笔作答,累计评估538天,共计完成1883次评估,整体遵从率90.9%)。根据自杀意念的频率、强度及变异性特征,利用潜剖面分析识别自杀意念的亚型数量。结果:当自杀意念亚型数量为5时,模型拟合指标最优。亚型差异主要表现在自杀意念平均强度和变异性上,高均值-高变异性的亚型在既往自杀未遂次数上显著高于其它亚型。结论:在有自杀史的精神科住院病人中,自杀意念具有5种不同的亚型,分别是中均值-高变异性亚型、中均值-低变异性亚型、高均值-中变异性亚型、低均值-中变异性亚型、高均值-高变异性亚型,其中高均值-高变异性亚型可能存在较高的自杀风险。Objective: Explore whether there exist subtypes of suicidal ideation in Chinese psychiatric inpatients with a history of suicide. Methods: Selected by convenient sampling method, 27 inpatients with a history of suicide in one psychiatric hospital in Tianjin were arranged to participate in one ecological momentary assessment of suicidal ideation, which was randomly assessed four times a day during natural hospitalization(ranging from 1 to 28 days, using real-time paperpen answering method, total assessment days=538, total completed assessment times=1883, overall compliance rate=90.9%). Based on the frequency, intensity and variability of suicidal ideation, the number of suicidal ideation subtype was identified by latent profile analysis. Results: When the number of suicidal ideation subtype was five, the model fitting index was the best. The difference of subtypes was mainly manifested in the average intensity and variability of suicidal ideation. The subtype with high mean and high variability was significantly higher than other subtypes in the number of previous attempted suicide. Conclusion: Among Chinese psychiatric inpatients with a history of suicide, there exist five different subtypes of suicidal ideation named the subtype with medium mean and high variability, the subtype with medium mean and low variability, the subtype with high mean and medium variability, the subtype with low mean and medium variability,and the subtype with high mean and high variability respectively, where the subtype with high mean and high variability may be at higher suicide risk.
