摘  要:职业性慢性苯中毒可引起白细胞减少,但某些药物亦会引起白细胞降低。2020年贵阳市某职业病防治院接诊1例疑似职业性慢性苯中毒患者,接苯作业4年,平均每天约30 min。该患者于2018-09诊断为继发性肺结核并给予规范抗结核治疗,2019年于职业健康检查时发现白细胞降低并给予升白细胞治疗,结核治愈后停用抗结核药物半年监测白细胞绝对计数及中性粒细胞绝对计数逐渐升高,通过《职业病诊断通则》不能诊断该患者为职业性慢性苯中毒,应通过《职业禁忌证界定导则》判定为苯作业禁忌证。Occupational chronic benzene poisoning can cause leucopenia,but some drugs can also cause leucopenia.In 2020,an occupational disease prevention and control hospital in Guiyang received a patient with suspected occupational chronic benzene poisoning.The patient had exposed in benzene for 4 years,with an average of about 30 minutes per day per week.The patient was diagnosed with secondary pulmonary tuberculosis in September 2018 and was given standard anti tuberculosis treatment.In 2019,leucopenia was found during occupational health examination and leucocyte raising treatment was given.After tuberculosis was cured,anti tuberculosis drugs were stopped for half a year,and the absolute count of leukocytes and neutrophils increased gradually.The patient can not be diagnosed as occupational chronic benzene poisoning through the general guideline for diagnosis of occupational disease.It should be determined as benzene occupational contraindication through the guideline of identification of contraindication to job placement.
