成都市某农村地区残疾人签约家庭医生服务的影响因素研究

摘　　要：目的 调查了解成都市某农村地区残疾人家庭医生服务的签约情况,分析影响签约的因素,为促进残疾人家庭医生签约服务提供参考依据。方法 采用随机抽样的方法,于2018年12月—2019年5月对成都市某农村地区的残疾人进行问卷调查,共计调查495名。采用描述性统计方法分析调查对象的基本情况,采用logistic回归分析残疾人签约家庭医生服务的影响因素。结果 调查残疾人家庭医生服务签约率为74.7%。二元logistic回归分析显示,年龄在60~69岁(OR=2.846,95%CI:1.304~6.209)以及70岁及以上(OR=2.649,95%CI:1.060~6.618)、自评残疾严重(OR=0.314,95%CI:0.171~0.574)、近2年利用过基层医疗卫生机构服务(OR=21.527,95%CI:7.147~64.840)、听说过家庭医生签约服务(OR=43.918,95%CI:22.984~83.921)是残疾人签约家庭医生服务的影响因素。结论 调查地区残疾人家庭医生服务签约率未达到标准要求。建议加大家庭医生签约服务的宣传力度,加强基层医疗卫生机构与社区和街道的合作,注重基层医疗卫生机构自身服务能力的提高,在残疾居民中形成值得信赖的口碑,树立良好形象,有效推进残疾人家庭医生签约服务,为残疾人健康提供良好保障。Objective To investigate and understand the contracting situation of family doctor services for people with disabilities in a rural area of Chengdu, analyze the factors affecting contracting, and provide a reference basis for promoting family doctor contracting services for people with disabilities. Methods Using a random sampling method, a questionnaire survey was conducted from December 2018 to May 2019 on a total of 495 people with disabilities in a rural area of Chengdu.Descriptive statistics were used to analyze the basic conditions of the survey respondents, and logistic regression was used to analyze the influencing factors of disabled people signing up for family doctor services. Results The contracting rate of family doctor services for the surveyed disabled people was 74.7%. Binary logistic regression analysis showed that people aged 60 to69 years(OR=2.846, 95%CI: 1.304-6.209) and 70 years and older(OR=2.649, 95%CI: 1.060-6.618), with severe self-rated disability(OR=0.314, 95%CI: 0.171-0.574), who had recently 2 years(OR=21.527, 95%CI: 7.147-64.840), and having heard of the family doctor contracted service(OR=43.918, 95%CI: 22.984-83.921) were the influencing factors for people with disabilities to sign up for family doctor services. Conclusion The contracting rate of family doctor services for people with disabilities in the survey area did not meet the standard requirements. It is recommended to increase the publicity of family doctor contracting service, strengthen the cooperation between primary health care institutions and communities and streets, focus on the improvement of primary health care institutions’ own service capacity, form a trustworthy reputation among disabled residents, and establish a good image to effectively promote family doctor contracting service for disabled people and provide good protection for their health.
