摘  要:目的研究老年2型糖尿病患者血清前神经降压肽(PNT)、白细胞介素-17A(IL-17A)水平与糖尿病周围神经病变(DPN)的关系。方法选取西安大兴医院内分泌代谢科2019年1月至2020年1月收治的老年2型糖尿病患者120例进行回顾性分析,按照是否合并DPN分为DPN组(n=50)和无DPN组(n=70)。比较两组患者入院后次日的血清PNT、IL-17A、空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)、正中神经运动传导速度(MN-MNCV)、正中神经感觉传导速度(MN-SNCV)、腓总神经运动传导速度(NPC-MNCV)、腓总神经感觉传导速度(NPC-SNCV)水平,并采用Pearson相关性分析DPN组患者的血清PNT、IL-17A水平与MN-MNCV、MN-SNCV、NPC-MNCV、NPC-SNCV的相关性。结果入院后次日,DPN组患者的PNT水平为(33.58±3.29)ng/mL,明显低于无DPN组的(39.96±3.73)ng/mL,IL-17A、FPG、2 hPG、HbA1c水平分别为(4.05±1.34)pg/mL、(8.37±0.98)mmol/L、(9.23±0.88)mmol/L、(8.16±0.72)%,明显高于无DPN组的(2.39±0.47)pg/mL、(7.12±0.85)mmol/L、(8.04±0.82)mmol/L、(7.26±0.69)%,差异均有统计学意义(P<0.05);无DPN组患者的MN-MNCV、MN-SNCV、NPC-MNCV、NPC-SNCV水平分别为(43.16±6.28)m/s、(43.21±6.22)m/s、(46.95±6.31)m/s、(46.33±6.29)m/s、明显优于DPN组的(37.84±5.03)m/s、(37.76±5.10)m/s、(41.28±6.59)m/s、(40.88±6.46)m/s,差异均有统计学意义(P<0.05);Pearson相关性分析结果显示,DPN组患者的MN-MNCV、MN-SNCV、NPC-MNCV、NPC-SNCV与PNT均呈正相关(r=0.496、0.512、0.509、0.522,P<0.05),与IL-17A呈负相关(r=-0.534、-0.519、-0.528、-0.562,P<0.05)。结论老年2型糖尿病患者的血清PNT、IL-17A水平与DPN密切相关,临床上需关注老年患者PNT、IL-17A水平,以预防DPN发生。Objective To investigate the relationship between the levels of proneurotensin(PNT),interleukin-17A(IL-17A)and diabetic peripheral neuropathy(DPN)in elderly patients with type 2 diabetes mellitus.Methods A total of 120 elderly patients with type 2 diabetes mellitus who were admitted to the Department of Endocrinology and Metabolism,Xi'an Daxing Hospital from January 2019 to January 2020 were retrospectively analyzed,which were divided into DPN group(n=50)and non-DPN group(n=70)according to whether there was DPN.Serum PNT,IL-17A,fasting blood glucose(FPG),2 h postprandial blood glucose(2 hPG),glycosylated hemoglobin(HbA1c),median nerve motor conduction velocity(MN-MNCV),median nerve sensory conduction velocity(MN-SNCV),peroneal nerve motor conduction velocity(NPC-MNCV),and total peroneal nerve sensory conduction velocity(NPC-SNCV)were compared between the two groups on the second day after admission.Pearson correlation analysis was used to analyze the correlation between serum PNT and IL-17A levels and MN-MNCV,MN-SNCV,NPC-MNCV and NPC-SNCV in DPN group.Results On the second day after admission,the level of PNT in DPN group was(33.58±3.29)ng/mL,significantly lower than(39.96±3.73)ng/mL in DPN group(P<0.05).IL-17a,FPG,2 hPG,and HbA1c levels were(4.05±1.34)pg/mL,(8.37±0.98)mmol/L,(9.23±0.88)mmol/L,(8.16±0.72)%,respectively,significantly higher than(2.39±0.47)pg/mL,(7.12±0.85)mmol/L,(8.04±0.82)mmol/L,(7.26±0.69)%in DPN group(P<0.05).The levels of MN-MNCV,MN-SNCV,NPC-MNCV,and NPC-SNCV in non-DPN group were(43.16±6.28)m/s,(43.21±6.22)m/s,(46.95±6.31)m/s,(46.33±6.29)m/s,respectively,significantly better than(37.84±5.03)m/s,(37.76±5.10)m/s,(41.28±6.59)m/s,(40.88±6.46)m/s in DPN group(P<0.05).Pearson correlation analysis showed that MN-MNCV,MN-SNCV,NPC-MNCV,and NPC-SNCV were positively correlated with PNT in DPN group(r=0.496,0.512,0.509,0.522,P<0.05),but negatively correlated with IL-17A(r=-0.534,-0.519,-0.528,-0.562,P<0.05).Conclusion The levels of PNT and IL-17A in elderly patients with type 2 diabetes
