阴式彩超在异位妊娠早期诊断中的应用

摘　　要：目的比较经阴道和经腹部彩色三维多普勒超声用于异位妊娠早期诊断的临床价值。方法选取2018年1月—2020年9月于医院就诊并经手术或通过病理活检所确诊的128例异位妊娠患者,分别运用经腹部彩色三维多普勒超声(transabdominal color three-dimensional Doppler ultrasound,TACDS)与经阴道彩色三维多普勒超声(transvaginal color three-dimensional Doppler ultrasound,TVCDS)检查,以术后病理结果为依据,通过比较两种三维彩色超声的临床诊断准确率,探讨这两种超声诊断方法的临床价值。结果经手术或病理活检确诊的128例异位妊娠患者,经腹超声检查诊断符合率为77.34%(99/128),经阴道超声诊断符合率为92.19%(118/128),差异有统计学意义(P<0.05)。同时,在附件区包块、盆腔积液、宫腔内假孕囊、胚芽检出及原始心血管搏动等早期异位妊娠典型超声影像学表现的检出率方面,经阴道彩色三维多普勒超声均优于经腹部彩色三维多普勒超声,差异有统计学意义(P<0.05)。结论经阴道三维彩色超声对异位妊娠的早期诊断的准确率明显高于经腹部三维彩色超声的诊断率,实用价值较高。Objective To compare the clinical value of transvaginal and transabdominal color three-dimensional Doppler ultrasound in the early diagnosis of ectopic pregnancy.Methods A total of 128 patients with ectopic pregnancy in our hospital from January 2018 to September 2020 were selected and applied transabdominal color three-dimensional Doppler ultrasound(TACDS)and transvaginal color three-dimensional Doppler Ultrasound(TVCDS)examination respectively,based on the postoperative pathological results,compared the clinical diagnostic accuracy of two three-dimensional color ultrasounds,and explored the clinical value of these two ultrasound diagnostic methods.Results In 128 cases of ectopic pregnancy diagnosed by surgery or pathological biopsy,the diagnostic coincidence rate of transabdominal ultrasound was 77.34%(99/128);the coincidence rate of transvaginal ultrasound was 92.19%(118/128),the difference was statistically significant(P<0.05).At the same time,transvaginal ultrasound was superior to transabdominal ultrasound in the detection rate of typical ultrasound imaging manifestations of early ectopic pregnancy such as germ reflex,primitive cardiovascular pulsation,adnexal mass,pelvic effusion,intrauterine pseudo-gestational sac,the difference was statistically significant(P<0.05).Conclusion The accuracy rate of transvaginal three-dimensional color ultrasound in the early diagnosis of ectopic pregnancy is significantly higher than that of transabdominal threedimensional color ultrasound,which is of higher practical value.
