清热解毒方治疗放射性肺炎急性期(热毒壅肺证)的临床观察

摘　　要：目的:观察清热解毒方治疗放射性肺炎急性期(热毒壅肺证)的临床疗效。方法:将80例急性期放射性肺炎患者随机分为治疗组和对照组各40例,对照组患者予基础治疗及注射用甲泼尼龙琥珀酸钠激素冲击治疗联合抗感染治疗,治疗组患者在对照组基础上予清热解毒方中药颗粒剂口服,连续治疗15 d,观察两组患者急性放射性肺炎分级(RTOG制定)、宗氏焦虑评分(SAS评分)、KPS评分、中医证候评分、不良反应,并评定疗效。结果:治疗组显效率为30.00%(12/40),对照组显效率为17.50%(7/40);治疗组患者疗效优于对照组,差异有统计学意义(P<0.05)。两组患者治疗后RTOG分级均较治疗前改善(P<0.05),且治疗组患者治疗后RTOG分级优于对照组,差异有统计学意义(P<0.05)。两组患者治疗后SAS评分、中医证候评分均较治疗前降低,KPS评分均较治疗前升高,差异均有统计学意义(P<0.05);治疗后两组比较,治疗组患者SAS评分、中医证候评分均低于对照组,KPS评分高于对照组,差异均有统计学意义(P<0.05)。对照组不良反应发生率为27.50%(11/40),治疗组不良反应发生率为22.50%(9/40);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:清热解毒方联合西医疗法治疗放射性肺炎急性期(热毒壅肺证)有良好疗效。Objective: To observe the clinical efficacy of Qingre Jiedu prescription in the treatment of acute radiation pneumonia(syndrome of heat toxin obstructing lung). Methods: A total of 80 patients with acute radiation pneumonia were randomly divided into a treatment group and a control group, with 40 cases in each group. The patients in the control group were treated with basic treatment and methylprednisolone sodium succinate hormone shock therapy combined with anti infection treatment. The patients in the treatment group were treated with Qingre Jiedu prescription traditional Chinese medicine granules for 15 days on the basis of the control group.The grade of acute radiation pneumonia(RTOG), SAS(Zong’s anxiety score), KPS score, TCM syndrome score and adverse reactions were observed in the two groups, and the curative effect was evaluated. Results: The effective rate in the treatment group was 30.00%(12/40), and the effective rate in the control group was 17.50%(7/40). The curative effect of the treatment group was better than that of the control group(P<0.05). The RTOG grade of the two groups after treatment was improved compared with that before treatment(P <0.05), and the RTOG grade of the treatment group was better than that of the control group(P<0.05). After treatment, the SAS score and TCM syndrome score of the two groups were lower than those before treatment, and the KPS score was higher than that before treatment(P<0.05). After treatment, the SAS score and TCM syndrome score of the treatment group were lower than those of the control group, and the KPS score was higher than that of the control group(P<0.05). The incidence of adverse reactions was 27.50%(11/40) in the control group and 22.50%(9/40)in the treatment group. There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion: Qingre Jiedu prescription combined with western medicine has a good effect in the treatment of acute radiation pneumonia(syndrome of heat toxin obstructing lung).
