摘 要:目的:分析经颅多普勒血流动力学参数与中医体质辨识的相关性,探讨经颅多普勒超声对中医体质辨识的诊断意义。方法:选取北京中医药大学东直门医院门诊患者51例及健康体检者13人。当日进行经颅多普勒血流超声检查和中医体质量表评测,计算颅内14个血管部位的S2峰时流速VS2和S1峰至S2峰的血流加速度AS2-1,分析不同中医体质与血流动力学参数的关系。结果:VS2-RPCA与阳虚质(r=-0.355,P <0.05)、湿热质(r=-0.400,P<0.05),与气郁质(r=-0.360,P<0.05)呈负相关;VS2-LPCA与湿热质呈负相关(r=-0.481,P<0.01);痰湿质与AS2-1-RCS呈负相关(r=-0.541,P<0.01);湿热质与AS2-1-RMCA(r=-0.360,P<0.05)、AS2-1-RCS(r=-0.507,P<0.01)、AS2-1-RVA(r=-0.359,P<0.05)呈负相关;血瘀质与AS2-1-LICA呈正相关(r=0.481,P<0.01)。结论:经颅多普勒超声血流动力学参数对中医体质的判别具有一定意义,且AS2-1值较S2峰时流速对中医体质辨识和临床早期干预有重要意义。Objective: To analyze the correlation between transcranial Doppler hemodynamic parameters and TCM constitution identification, and to explore the diagnostic significance of transcranial Doppler ultrasound in TCM constitution identification. Methods: A total of 51 outpatients and 13 healthy people were selected from Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine. Transcranial Doppler blood flow ultrasound examination and TCM constitution scale evaluation were carried out on the same day. The S2 peak velocity VS2and the blood flow acceleration AS2-1from S1 peak to S2 peak of 14 intracranial blood vessels were calculated, and the relationship between different TCM constitutions and hemodynamic parameters was analyzed.Results: VS2-RPCA was negatively correlated with Yang deficiency constitution(r =-0.355, P <0.05), damp heat constitution(r =-0.400, P<0.05) and Qi depression constitution(r =-0.360, P<0.05). VS2-LPCA showed negative correlation with damp heat constitution(r=-0.481, P<0.01). Phlegm dampness constitution showed negative correlation with AS2-1-RCS(r=-0.541, P<0.01). Damp heat constitution was negatively correlated with AS2-1-RMCA(r=-0.360, P<0.01)、AS2-1-RCS(r=-0.507, P<0.01) and AS2-1-RVA(r=-0.359, P<0.05). Blood stasis constitution showed positive correlation with AS2-1-LICA(r=0.481, P<0.01). Conclusion: Transcranial Doppler ultrasound hemodynamic parameters have certain significance for the identification of TCM constitution, and the AS2-1value compared with S2 peak flow velocity is of great significance for TCM constitution identification and early clinical intervention.
