摘　　要：目的探讨H型高血压患者基质金属蛋白酶-9(MMP-9)、同型半胱氨酸(Hcy)水平与认知损害的关系。方法回顾性选取2019年3月至2021年5月在海口市人民医院就诊的120例H型高血压患者,计为H型高血压组,入院均接受简易智力状态检查量表(MMSE)、MMP-9、Hcy及相关指标检查。统计认知损害发生率,并依据有无认知损害分为认知正常组和认知损害组,对两组间的检查结果进行比较分析。分析MMSE评分与MMP-9、Hcy水平的相关性。并分析H型高血压患者发生认知损害的影响因素。结果本组120例H型高血压患者认知损害发生率为43.33%(52/120),轻度、中度和重度认知损害分别34例、15例和3例。认知损害组年龄、吸烟史、受教育年限和血清MMP-9、Hcy水平与认知正常组比较,差异有统计学意义(P<0.05)。中重度认知损害患者血清MMP-9、Hcy水平高于轻度认知损害患者,差异有统计学意义(P<0.05)。H型高血压患者血清MMP-9、Hcy水平与MMSE评分均呈负相关(r=-0.776、-0.736,P<0.05)。多因素Logistic回归发现,血清MMP-9(OR=1.314,95%CI:1.095~1.533)、Hcy(OR=1.527,95%CI:1.204~1.850)水平升高均是H型高血压患者发生认知损害的危险因素(P<0.05)。结论H型高血压患者易发生认知损害,且血清MMP-9、Hcy水平与认知损害发生及损害程度密切相关。Objective To investigate the relationship between the levels of matrix metalloproteinase-9(MMP-9),homocysteine(Hcy)and cognitive impairment in patients with type H hypertension.Methods One hundred-twenty patients with H-type hypertension treated in Haikou People's Hospital from March 2019 to may 2021 were recorded as H-type hypertension group.They were admitted to hospital and examined by simple mental state examination scale(MMSE),MMP-9,Hcy and related indexes.The incidence of cognitive impairment was counted and divided into cognitive normal group and cognitive impairment group according to whether there was cognitive impairment.The examination results between the two groups were compared and analyzed.The correlation of MMSE score with MMP-9 and Hcy levels was analyzed.The influencing factors of cognitive impairment in H-type hypertension patients were analyzed.Results The incidence of cognitive impairment in 120 patients with H-type hypertension was 43.33%(52/120).There were 34 cases of mild,15 cases of moderate and 3 cases of severe cognitive impairment,respectively.There were significant differences in age,smoking history,years of education,serum MMP-9 and Hcy levels between cognitive impairment group and cognitive normal group(P<0.05).The levels of serum MMP-9 and Hcy in patients with moderate and severe cognitive impairment were significantly higher than those in patients with mild cognitive impairment(P<0.05).The levels of serum MMP-9 and Hcy in patients with type H hypertension were negatively correlated with MMSE score(r=-0.776,-0.736,P<0.05).Multivariate logistic regression showed that the increase of serum MMP-9(OR=1.314,95%CI:1.095-1.533)and Hcy(OR=1.527,95%CI:1.204-1.850)were risk factors for cognitive impairment in patients with type H hypertension(P<0.05).Conclusion Patients with H-type hypertension are prone to cognitive impairment,and the levels of serum MMP-9 and Hcy are closely related to the occurrence and degree of cognitive impairment.
