摘 要:目的:分析妊娠期糖尿病(gestational diabetes mellitus,GDM)患者血清脂蛋白相关磷脂酶A2(lipoprotein associated phospholipase A2,Lp-PLA2)和胎盘亮氨酸氨基肽酶(placental leucine aminopeptidase,P-LAP)水平与糖脂代谢指标的相关性。方法:选取2019年11月-2021年6月深圳市龙华区人民医院确诊GDM患者156例为GDM组,同期非GDM健康孕妇117例为对照组。分别检测血清Lp-PLA2、P-LAP、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)及高密度脂蛋白胆固醇(HDL-C)水平,并计算出胰岛素抵抗指数(HOMA-IR)。结果:GDM组Lp-PLA2水平为(281.06±29.41)ng/mL,明显高于对照组的(142.73±23.60)ng/mL,而P-LAP水平为(495.27±186.58)pg/mL,明显低于对照组的(1019.67±205.95)pg/mL,差异均有统计学意义(P<0.05)。GDM组患者FPG、HbA1c、FINS、HOMA-IR、TC、TG及LDL-C水平分别为(6.37±1.42)mmol/L、(7.12±1.54)%、(13.02±5.21)μIU/mL、(2.31±0.73)、(4.23±0.71)mmol/L、(6.57±0.92)mmol/L及(3.80±0.54)mmol/L,均明显高于对照组的(4.09±0.65)mmol/L、(3.82±0.59)%、(7.89±2.65)μIU/mL、(1.49±0.48)、(3.10±0.47)mmol/L、(4.66±0.54)mmol/L及(2.59±0.26)mmol/L,而HDL-C水平为(1.24±0.26)mmol/L,明显低于对照组的(1.87±0.38)mmol/L,差异均有统计学意义(P<0.05)。经Pearson相关性分析,Lp-PLA2与FPG、HbA1c、FINS、HOMA-IR、TC、TG及LDL-C水平均呈正相关,与HDL-C水平呈负相关,而P-LAP与FPG、HbA1c、FINS、HOMA-IR、TC、TG及LDL-C水平均呈负相关,与HDL-C水平呈正相关(P<0.05)。结论:GDM患者Lp-PLA2水平明显升高,而P-LAP水平降低,且与糖脂代谢指标具有一定的相关性,其可能是通过影响糖脂代谢而在发生GDM中发挥重要作用的。Objective:To analyze the levels of Lipoprotein associated phospholipase A2(Lp-PLA2)and placental leucine aminopeptidase(P-LAP)in gestational diabetes mellitus(GDM)patients and their correlation with glycolipid metabolism.Method:A total of 156 patients diagnosed with GDM in Shenzhen Longhua District People’s Hospital from November 2019 to June 2021 were selected as GDM group,and 117 healthy non-GDM pregnant women were selected as control group during the same period.The levels of Lp-PLA2,P-LAP,blood glucose(FPG),glycated hemoglobin(HbA1c),fasting insulin(FINS),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)and high density lipoprotein cholesterol(HDL-C)were detected,the insulin resistance index(HOMA-IR)was calculated.Result:The level of Lp-PLA2 in GDM group was(281.06±29.41)ng/mL,significantly higher than(142.73±23.60)ng/mL of the control group,while the level of P-LAP was(495.27±186.58)pg/mL in GDM group,significantly lower than(1019.67±205.95)pg/mL of the control group,the differences were statistically significant(P<0.05).The levels of FPG,HbA1c,FINS,HOMA-IR,TC,TG and LDL-C were(6.37±1.42)mmol/L,(7.12±1.54)%,(13.02±5.21)μIU/mL,(2.31±0.73),(4.23±0.71)mmol/L,(6.57±0.92)mmol/L and(3.80±0.54)mmol/L in GDM group,respectively,significantly higher than(4.09±0.65)mmol/L,(3.82±0.59)%,(7.89±2.65)μIU/mL,(1.49±0.48),(3.10±0.47)mmol/L,(4.66±0.54)mmol/L and(2.59±0.26)mmol/L,while the level of HDL-C was(1.24±0.26)mmol/L of the control group,which was significantly lower than(1.87±0.38)mmol/L of the control group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that Lp-PLA2 was positively correlated with FPG,HbA1c,FINS,HOMA-IR,TC,TG and LDL-C,negatively correlated with HDL-C,while P-LAP was negatively correlated with FPG,HbA1c,FINS,HOMA-IR,TC,TG and LDL-C,but positively correlated with HDL-C(P<0.05).Conclusion:In GDM patients,the level of Lp-PLA2 is significantly increased,while the level of P-LAP is decreased,and it is correlat
