摘 要:目的:探讨锁定加压钢板(LCP)结合微创经皮钢板固定技术(minimally invasive percutaneous plate osteosynthesis,MIPPO)治疗胫骨远端骨折的优良率及对愈合时间、负重时间、bFGF水平及BMP-2水平的影响。方法:选取2018年7月-2020年7月佳木斯市中医院收治的86例胫骨远端骨折患者进行回顾性分析,并根据治疗方式的不同分组。对照组进行传统钢板固定治疗,研究组使用LCP联合MIPPO技术治疗。观察比较两组的优良率;两组术后的骨折愈合时间及负重时间;两组手术前后的血清纤维细胞生长因子(bFGF)水平及骨形成蛋白(BMP-2)水平变化差异。结果:研究组优良率显著高于对照组(P<0.05)。研究组骨折愈合时间及负重时间均显著短于对照组(P<0.05)。术后,研究组血清bFGF及BMP-2水平均显著高于对照组(P<0.05)。结论:针对胫骨远端骨折患者使用LCP联合MIPPO技术治疗,可显著提升治疗优良率,缩短骨折愈合及负重时间,提高BMP-2及bFGF水平。Objective:To investigate the excellent and good rate of locking compression plate (LCP) combined with minimally invasive percutaneous plate osteosynthesis (MIPPO) in the treatment of distal tibial fractures and its effects on healing time,weight-bearing time,bFGF level and BMP-2 level.Method:A total of 86 patients with distal tibial fractures admitted to Jiamusi Hospital of Traditional Chinese Medicine from July 2018 to July 2020 were selected for retrospective analysis and grouped according to different treatment methods.The control group was treated with traditional plate fixation,and the study group was treated with LCP combined with MIPPO technology.The excellent and good rates of the two groups were observed and compared;the fracture healing time and weight-bearing time of the two groups after surgery were observed and compared;the changes in serum fibroblast growth factor (bFGF) levels and bone morphogenetic protein (BMP-2) levels between the two groups before and after surgery were observed and compared.Result:The excellent and good rate of the study group was significantly higher than that of the control group (P<0.05).The fracture healing time and weight-bearing time in the study group were significantly shorter than those in the control group (P<0.05).Serum bFGF levels and BMP-2 levels after surgery were significantly higher than those in the control group (P<0.05).Conclusion:The use of LCP combined with MIPPO technology for treatment of distal tibial fractures can significantly improve the excellent and good rate of treatment,shorten fracture healing and weight-bearing time,and increase BMP-2 and bFGF levels.
