消渴肾宝丸治疗早期糖尿病肾脏疾病气阴两虚、瘀血内阻证的临床观察

摘　　要：目的:探讨早期糖尿病肾脏疾病(DKD)气阴两虚、瘀血内阻证应用消渴肾宝丸治疗的临床效果。方法:选择2020年1月-2021年6月于九江市中医医院治疗的100例早期DKD患者,采用随机数字表法分为两组,每组50例。对照组给予基础治疗,观察组在此基础上加用消渴肾宝丸治疗。对比两组临床疗效、血糖水平[空腹血糖(FBG)、餐后2 h血糖(2 h PG)和糖化血红蛋白(HbA1c)]、肾功能[尿素氮(BUN)、血肌酐(Scr)、尿微量白蛋白(mALB)、mALB/Scr比值(ACR)]及不良反应。结果:治疗后,观察组临床总有效率(98.00%)高于对照组(82.00%),FBG(6.21±1.76)mmol/L、2 h PG(8.10±2.01)mmol/L、HbA1c(6.12±1.04)%、BUN(6.68±1.37)μmol/L、Scr(69.21±8.62)mmol/L、mALB(42.19±6.37)mg/L、尿ACR(60.15±10.11)mg/g,均低于对照组的(7.12±2.24)mmol/L、(9.25±2.33)mmol/L、(6.98±1.22)%、(7.78±1.31)μmol/L、(81.46±9.50)mmol/L、(78.37±8.44)mg/L、(93.41±20.24)mg/g,差异均有统计学意义(P<0.05)。治疗期间两组未见明显不良反应。结论:早期DKD气阴两虚、瘀血内阻证患者应用消渴肾宝丸治疗能降低血糖水平,改善肾功能,且安全性较高,利于预后。Objective:To investigate the clinical effect of Xiaoke Shenbao Pills in the treatment of early diabetic kidney disease (DKD) with deficiency of Qi and Yin and internal obstruction of blood stasis.Method:A total of 100 patients with early DKD treated in Jiujiang Hospital of Traditional Chinese Medicine from January 2020 to June 2021 were selected and divided into two groups by random number table method,with 50 cases in each group.The control group was given basic treatment,based on this,the observation group was added Xiaoke Shenbao Pills.The clinical efficacy,blood glucose level[fasting blood glucose (FBG),2 h postprandial blood glucose(2 h PG) and glycosylated hemoglobin (HbA1c)],renal function[urea nitrogen (BUN),serum creatinine (Scr),urea microalbumin (mALB),mALB/Scr ratio (ACR)]and adverse reactions were compared between the two groups.Result:After treatment,the total clinical effective rate of observation group (98.00%) was higher than that of control group (82.00%),FBG (6.21±1.76) mmol/L,2 h PG (8.10±2.01) mmol/L,HbA1c (6.12±1.04) %,BUN (6.68±1.37) μmol/L,Scr (69.21±8.62) mmol/L,mALB (42.19±6.37) mg/L and urine ACR (60.15±10.11) mg/gin the observation group were lower than (7.12±2.24)) mmol/L,(9.25±2.33) mmol/L,(6.98±1.22) %,(7.78±1.31) μmol/L,(81.46±9.50) mmol/L,(78.37±8.44) mg/L,(93.41±20.24) mg/g in the control group,with statistical differences (P<0.05).There were no obvious adverse reactions in the two groups during treatment.Conclusion:The treatment of Xiaoke Shenbao Pills in early DKD patients with deficiency of Qi and Yin and internal obstruction of blood stasis can reduce blood glucose level,improve renal function,with high safety and good prognosis.
