切开复位T型钛板内固定术治疗Barton骨折的疗效观察

摘　　要：目的探讨切开复位T型钛钣内固定治疗Barton骨折的临床效果及安全性。方法选择2016年1月至2020年1月华北医疗健康集团邢台总医院骨科治疗的160例Barton骨折患者进行回顾性研究,根据治疗方法不同分为对照组和研究组,每组各80例。对照组患者采用外固定支架治疗,研究组患者采用切开复位T型钛钣内固定术治疗。术后6个月,采用Mehara功能评价标准对两组患者的疗效进行评价;并比较两组患者的手术时间、骨性愈合时间、掌倾角、尺偏角、桡骨高度恢复情况及术后并发症发生情况。结果研究组患者治疗优良率为90.00%,明显高于对照组(72.50%),差异有统计学意义(P<0.05)。研究组患者的手术时间为(1.47±0.21)h,明显长于对照组[(1.24±0.16)h],但研究组患者的骨性愈合时间为(8.31±0.89)周,明显短于对照组[(9.47±1.13)周],差异均有统计学意义(P<0.05)。术后6个月,研究组患者的掌倾角、尺偏角、桡骨高度分别为(12.43±2.36)°、(22.69±2.48)°、(10.89±1.84)mm,均大于对照组患者[(10.98±2.17)°、(18.74±2.37)°、(9.76±1.79)mm],差异均有统计学意义(P<0.05)。研究组患者术后并发症发生率为2.50%,明显低于对照组患者(12.50%),差异有统计学意义(P<0.05)。结论采用切开复位T型钛钣内固定术治疗Barton骨折疗效显著,骨性愈合快,术后功能恢复良好,且术后并发症少,安全性高。Objective To investigate the clinical effect and safety of open reduction and internal fixation with T-type titanium plate in the treatment of Barton fracture.Methods A retrospective study of 160 patients with Barton fracture treated in Department of Orthopedic,Xingtai General Hospital,North China Medical and Health Group from January 2016 to January 2020 was selected for a retrospective study.According to different treatment methods,they were divided into control group and study group,with 80 cases in each group.The patients in the control group were treated with external fixation brackets,and the patients in the study group were treated with open reduction and internal fixation with T-type titanium plate.The efficacy of the two groups of patients was evaluated by the Mehara function evaluation standard 6 months after the operation.The operation time,bone healing time,palm inclination,ulnar deflection,radius recovery and postoperative complications were compared between the two groups.Results The excellent and good rate of treatment in the study group was 90.00%,which was significantly higher than that in the control group(72.50%),the difference was statistically significant(P<0.05).The operation time of the study group was(1.47±0.21)h,which was significantly longer than that of the control group[(1.24±0.16)h],however,the bone healing time of patients in the study group was(8.31±0.89)weeks,which was significantly shorter than that of the control group(9.47±1.13)weeks,and the differences were statistically significant(P<0.05).At 6 months after the pperation,the palm inclination angle,ulnar deflection angle,radius height of the study group were(12.43±2.36)°,(22.69±2.48)°,(10.89±1.84)mm,which were larger than those of the control group[(10.98±2.17)°,(18.74±2.37)°,(9.76±1.79)mm],and the differences were statistically significant(P<0.05).The postoperative complication rate of the study group was 2.50%,which was significantly lower than of the control group(12.50%),the difference was statistically signif
