12导联常规心电图联合超声心动图检查诊断早期复极综合征的临床价值

摘　　要：目的分析12导联常规心电图联合超声心动图检查诊断早期复极综合征的临床价值。方法回顾性分析2018年10月至2019年12月本院收治的80例疑似早期复极综合征患者的临床资料,后期治疗确诊早期复极综合征48例,非早期复极综合征32例。所有患者均行12导联常规心电图检查、超声心动图检查,分析单一12导联常规心电图检查、超声心动图检查和联合检查对早期复极综合征的诊断价值,以后期治疗确诊结果为标准,比较不同诊断方法的灵敏度、特异度、准确率。结果12导联常规心电图检查诊断早期复极综合征的灵敏度为81.25%,特异度为84.38%,准确率为82.50%;超声心动图检查诊断早期复极综合征灵敏度为85.42%,特异度为87.50%,准确率为86.25%;联合检查诊断早期复极综合征灵敏度为93.75%,特异度为96.88%,准确率为95.00%;联合检查诊断早期复极综合征的灵敏度、特异度、准确率均高于12导联常规心电图和超声心动图单一检查,差异有统计学意义(P<0.05)。结论采用12导联常规心电图检查联合超声心动图检查的灵敏度、特异度、准确率更高,可降低漏诊率,具有推广与借鉴价值。Objective To analyze the clinical value of 12-lead routine electrocardiogram combined with echocardiography in the diagnosis of early repolarization syndrome.Methods The clinical data of 80 patients with suspected early repolarization syndrome admitted to our hospital from October2018 to December 2019 were retrospectively analyzed.A total of 48 patients with early repolarization syndrome and 32 patients with non-early repolarization syndrome were diagnosed by later treatment.All patients underwent 12-lead routine electrocardiogram and echocardiography,the diagnostic value of single 12-lead routine electrocardiogram,echocardiography and combined examination for early repolarization syndrome was analyzed.The sensitivity,specificity and accuracy of different diagnostic methods were compared based on the diagnosis results of later treatment.Results The sensitivity,specificity and accuracy of 12-lead routine electrocardiogram in diagnosing early repolarization syndrome were 81.25%,84.38% and 82.50%,respectively.The sensitivity,specificity and accuracy of echocardiography in early repolarization syndrome were 85.42%,87.50% and 86.25%,respectively.The sensitivity,specificity and accuracy of combined examination in the diagnosis of early repolarization syndrome were 93.75%,96.88% and 95.00% respectively.The sensitivity,specificity and accuracy of combined examination in the diagnosis of early repolarization syndrome were higher than those of 12-lead routine electrocardiogram and echocardiography,and the difference was statistically significant(P<0.05).Conclusion The sensitivity,specificity and accuracy of 12-lead routine electrocardiogram combined with echocardiography are higher,which can reduce the missed diagnosis rate,and has the value of popularization and reference.
