强心饮合补阳还五汤治疗慢性心力衰竭的效果及对患者血清Lp-PLA2、sSema4D、sICAM-1水平的影响

摘　　要：目的:探讨强心饮合补阳还五汤治疗慢性心力衰竭(CHF)的效果及对患者血清脂蛋白相关磷脂酶A2(Lp-PLA2)、可溶性Sema4D(sSema4D)、可溶性细胞间黏附分子1(sICAM-1)水平的影响。方法:选取2019年1月-2021年1月佳木斯市妇幼保健院收治的CHF患者124例为观察对象,按照随机数字表法将其分为对照组与研究组,各62例。对照组采用常规西医治疗,观察组在对照组基础上加用强心饮合补阳还五汤治疗。比较两组疗效、中医症状积分、左心室射血分数(LVEF)、左心室舒张期内径(LVDd)、心室短轴缩短率(FS)及血清Lp-PLA2、sSema4D、sICAM-1水平。结果:观察组总有效率为96.77%高于对照组的83.87%(P<0.05);治疗后,两组主症、次症症状积分均较治疗前降低,且观察组均低于对照组(P<0.05);治疗后,两组LVEF、FS均较治疗前升高,LVDd均较治疗前降低,且观察组LVEF、FS均高于对照组,LVDd低于对照组(P<0.05);治疗后,两组血清Lp-PLA2、sSema4D、sICAM-1水平均较治疗前降低,且观察组均低于对照组(P<0.05)。结论:强心饮合补阳还五汤能提高CHF治疗效果,缓解症状,逆转心室重构,改善心功能,其机制可能与降低Lp-PLA2、sSema4D、sICAM-1水平有关。Objective:To explore the effect of Qiangxinyin combined with Buyang Huanwu Decoction in the treatment of chronic heart failure(CHF)and its effect on serum lipoprotein-associated phospholipase A2(Lp-PLA2),soluble semaphorin 4D(sSema4D),and soluble intercellular adhesion molecule-1(sICAM-1)levels of patients.Method:A total of 124 CHF patients admitted to Jiamusi Maternity and Child Health Hospital from January 2019 to January 2021 were selected as the observation subjects,and they were divided into control group and study group according to random number table method,with 62 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Qiangxinyin combined with Buyang Huanwu Decoction on the basis of the control group.Efficacy,TCM symptom score,left ventricular ejection fraction(LVEF),left ventricular diastolic diameter(LVDd),ventricular short axis shortening rate(FS)and serum Lp-PLA2,sSema4D and sICAM-1 levels were compared between the two groups.Result:The total effective rate of observation group was 96.77%higher than 83.87%of control group(P<0.05).After treatment,the scores of primary and secondary symptoms in two groups were lower than those before treatment,and those of the observation group were lower than those of the control group(P<0.05).After treatment,LVEF and FS in two groups were higher than those before treatment,LVDd were lower than those before treatment,and LVEF and FS in observation group were higher than those of control group,LVDd was lower than that of control group(P<0.05).After treatment,the levels of serum Lp-PLA2,sSema4D and sICAM-1 in two groups were lower than those before treatment,and those in observation group were lower than those in control group(P<0.05).Conclusion:Qiangxinyin combined with Buyang Huanwu Decoction can improve the therapeutic effect of CHF,relieve symptoms,reverse ventricular remodeling and improve cardiac function,and the mechanism may be related to the decrease of Lp-PLA2,sSema4D and sICAM-1 levels.
