FMD及血清BigET-1、IgE对冠心病患者的诊断价值及其与病情严重程度的相关性

摘  要:目的:研究血管扩张功能(FMD)及血清血浆大内皮素-1(BigET-1)、免疫球蛋白E(IgE)对冠心病患者的诊断价值及其与病情严重程度的相关性。方法:选取2020年3月-2021年3月赣州市人民医院收治的72例冠心病患者为观察组,将同期70例健康体检者为对照组。检测两组血浆大内皮素-1(BigET-1)、免疫球蛋白E(IgE)及血管扩张功能(FMD)水平。采用工作特征曲线(ROC)分析BigET-1、IgE及FMD对CHD患病的诊断价值。按照根据Gensini评分将观察组分为轻度组、中度组和重度组。比较不同病情严重程度患者的BigET-1、IgE及FMD水平。采用Spearman相关性检验分析BigET-1、IgE及FMD与CHD患者病情严重程度的相关性。结果:观察组血清BigET-1、IgE水平均高于对照组,FMD水平低于对照组,差异均有统计学意义(P<0.05)。经ROC曲线分析,血清BigET-1、IgE水平及FMD对CHD均具有较高诊断价值,其AUC分别为0.888、0.998、0.951,敏感度分别是0.871、0.986、0.871,特异度分别是0.792、0.972、0.958。重度组BigET-1、IgE水平均高于轻、中度组,FMD水平均低于轻、中度组,且中度组BigET-1、IgE水平均高于轻度组,FMD水平低于轻度组,差异均有统计学意义(P<0.05)。Spearman相关性分析显示,BigET-1、IgE水平与CHD病情严重程度呈正相关,FMD与病情呈负相关(P<0.05)。结论:CHD患者病情严重程度与FMD及血清BigET-1、IgE水平密切相关,可有效评估CHD病情情况。Objective:To study the diagnostic value of vasodilatation function(FMD),serum BigET-1 and immunoglobulin E(IgE)in patients with coronary heart disease and their correlation with disease severity.Method:A total of 72 CHD patients admitted to Ganzhou People’s Hospital from March 2020 to March 2021 were selected as the observation group,and 70 healthy subjects during the same period were selected as the control group.Plasma levels of BigET-1(BigET-1),immunoglobulin E(IgE)and vasodilatation function(FMD)were detected.Operating characteristic curve(ROC)was used to analyze the diagnostic value of BigET-1,IgE and FMD for CHD.The observation group was divided into mild group,moderate group and severe group according to Gensini score.The levels of BigET-1,IgE and FMD in patients with different severity of disease were compared.Spearman correlation test was used to analyze the correlation between BigET-1,IgE and FMD and the severity of CHD patients.Result:The levels of serum BigET-1 and IgE in the observation group were higher than those in the control group,and the level of FMD in the observation group was lower than that in the control group,the differences were statistically significant(P<0.05).According to ROC curve analysis,serum BigET-1,IgE and FMD had high diagnostic value for CHD,with AUC of 0.888,0.998 and 0.951,sensitivity of 0.871,0.986 and 0.871,and specificity of 0.792,0.972 and 0.958,respectively.The levels of BigET-1 and IgE in the severe group were higher than those in the mild and moderate group,the levels of FMD in the severe group was lower than those in the mild and moderate group,and the levels of BigET-1 and IgE in the moderate group were higher than those in the mild group,and the level of FMD in the moderate group was lower than that in the mild group,the differences were statistically significant(P<0.05).Spearman correlation analysis showed that the levels of BigET-1 and IgE were positively correlated with the severity of CHD,and FMD was negatively correlated with the severity of CHD(P<0.05).Conc
