健脾除湿消浊汤治疗2型糖尿病合并高脂血症的效果观察及其对糖脂代谢和微炎症状态的影响

摘　　要：目的:观察自拟健脾除湿消浊汤辅助西药治疗2型糖尿病合并高脂血症患者的临床疗效及其对糖脂代谢和微炎症状态的影响。方法:80例2型糖尿病合并高脂血症脾虚湿困证患者随机分为两组,对照组予常规西药治疗,观察组加服健脾除湿消浊汤,连续服药12周后评价疗效;治疗前后对脾虚湿困证候主症进行评分,检测血糖(FBG、2 hPG、HbA1c)和血脂(TC、TG、LDL、HDL)指标,ELISA检测血清hs-CRP(高敏C反应蛋白)、IL-6(白介素-6)、IL-8(白介素-8)、TNF-α(肿瘤坏死因子-α)含量。结果:加服健脾除湿消浊汤观察组血糖、血脂达标率(95.0%、90.0%)明显高于对照组(70.0%、65.0%)(均P<0.05)。治疗12周后,两组患者脾虚湿困主症评分、FBG、2 hPG、HbA1c和TC、TG、LDL水平均明显降低,HDL水平明显升高(均P<0.05),血清hs-CRP、IL-6、IL-8、TNF-α含量明显减低(均P<0.05);观察组上述观察指标改善效果明显优于对照组(均P<0.05)。结论:自拟健脾除湿消浊汤辅助西药治疗2型糖尿病合并高脂血症患者可显著提高血糖、血脂达标率,减轻症状,调节糖脂代谢紊乱,抑制微炎症状态。Objective:To observe the curative effect of type 2 diabetes mellitus merged with hyperlipidaemia treated with Jianpi Chushi Xiaozhuo Decoction combined with Western medicine routine therapy and the regulation effects on blood glucose,lipid and micro inflammation.Methods:80 Cases with type 2 diabetes mellitus merged with hyperlipidaemia were divided into two groups at random,control group was treated with Western medicine routine therapy,observation group was treated Jianpi Chushi Xiaozhuo Decoction on the basis of control group,they were treated for 12 weeks,then the curative effect was evaluated,main symptoms score of dampness due to spleen deficiency syndrome were recorded,serum FBG,2 hPG,HbA1c and TC,TG,LDL,HDL were determined by automatic biochemical analyzer,ELISA was used to test serum hs-CRP,IL-6,IL-8,TNF-α.Results:The compliance rate of blood glucose and lipid in observation group were 95.0%and 90.0%,that were higher than those of control group that were70.0%,65.0%(P<0.05).The main symptoms score,serum FBG,2 hPG,HbA1c and TC,TG,LDL were lowered in the two groups after treatment,HDL was raised,serum hs-CRP,IL-6,IL-8,TNF-αlevels in the two groups were lowered(P<0.05).All above changes in the observation group were more significant than those in control group(P<0.05).Conclusion:Jianpi Chushi Xiaozhuo Decoction is helpful for raising compliance rate of blood glucose and lipid of type 2 diabetes mellitus merged with hyperlipidaemia patients treated with Western medicine routine therapy,relieve symptoms,regulate the disorder of glucose and lipid metabolism,inhibit inflammatory.
