隔药灸神阙穴治疗腹泻型肠易激综合征的疗效观察及其对血清5-HT、IL-10的影响

摘　　要：目的:观察隔药灸神阙穴治疗腹泻型肠易激综合征(IBS-D)患者的临床疗效及其对血清5-HT(5-羟色胺)、IL-10(白介素-10)的影响。方法:60例IBS-D患者以随机数字表法分为2组,每组各30例,观察组采取隔药灸神阙穴治疗,对照组给予双歧杆菌乳杆菌三联活菌片治疗,治疗4周后评估临床治疗效果,治疗前后分别采用胃肠症状自评量表(GSRS)、IBS症状严重程度量表(IBS-SSS)、IBS生活质量量表(IBS-QOL)进行评分,ELISA检测血清5-HT、IL-10含量。结果:观察组治疗4周后临床总有效率(93.3%)高于对照组(70.0%)(P<0.05);治疗后,2组患者GSRS评分、IBS-SSS评分较治疗前均降低(均P<0.05),IBS-QOL评分较治疗前均提高(P<0.05),血清5-HT水平明显降低(P<0.05),IL-10含量明显升高(P<0.05);观察组治疗后GSRS评分、IBS-SSS评分低于对照组(P<0.05),IBS-QOL评分高于对照组(P<0.05),血清5-HT水平低于对照组(P<0.05),IL-10高于对照组(P<0.05)。结论:IBS-D患者给予隔药灸神阙穴治疗临床疗效显著,有利于缓解胃肠道症状,减轻病情严重程度,提高患者生活质量;减轻肠道敏感性,抑制炎症反应是其部分作用机制。Objective:To observe the curative Effect of IBS-D patients treated with Herbs-partition Moxibustion at Shenque(CV8)Acupoint and serum 5-HT,IL-10 levels changes.Methods:60 Cases with IBS-D were divided into two groups according to random number table with each 30 cases,observation group was treated with Herbs-partition Moxibustion at Shenque Acupoint,control group was treated with Bifidobacterium and Lactobacillus Tablet,they were treated for 4 weeks,appraised the curative effect.Scores of GSRS,IBS-SSS and IBS-QOL were recorded and serum 5-HT,IL-10 contents were determined by ELISA.Results:The total effective rate of observation group was 93.3%,that was better than that of control group that was 70.0%(P<0.05);after treatment for 4 weeks,scores of GSRS,IBS-SSS of the two groups were lower significantly(P<0.05),IBS-QOL score was raised(P<0.05),serum 5-HT level was reduced and IL-10 level was raised(P<0.05);all above indexes changes in observation group were more significant than those in control group(P<0.05).Conclusion:Herbs-partition Moxibustion at Shenque Acupoint can effectively treat IBS-D,that is conducive to relieve gastrointestinal symptoms and severity,improve the quality of life;relieving intestinal sensitivity and inhibiting inflammatory reaction were part of its mechanism.
