情绪疏导联合亲情护理对老年血液透析患者的影响

摘　　要：目的观察情绪疏导联合亲情护理对老年血液透析患者的影响。方法选取我院2019年5月~2020年7月进行血液透析老年患者200例。按随机数字表法为对照组及观察组,每组100例。对照组给予情绪疏导,观察组在对照组护理上联合亲情护理模式。两组均护理4周后,观察两组患者护理前后焦虑及抑郁状态、治疗依从性及护理后自我管理能力。结果护理后观察组汉密尔顿抑郁量表(HAMD)评分(21.93±2.62)分、汉密尔顿焦虑量表(HAMA)评分为(20.17±3.06)分低于对照组(P<0.05);观察组饮食依从评分(27.82±4.92)分、饮水依从评分(19.74±3.91)分、用药依从评分(21.13±3.15)分、透析方案依从评分(17.36±2.45)分均高于对照组(P<0.05);观察组伙伴关系评分(15.22±3.16)分、问题解决评分(17.18±2.41)分、执行自我护理评分(21.71±2.84)分、情绪处理评分(11.45±2.41)分均优于对照组(P<0.05)。结论情绪疏导联合亲情护理模式应用对老年血液透析患者,可改善其不良情绪,提高治疗依从性。Objective To observe the effect of emotional counseling combined with kindred-like nursing on elderly patients undergoing hemodialysis.Methods A total of 200 elderly patients who underwent hemodialysis in the hospital were selected between May 2019 and July 2020,and were divided into control group and observation group according to the random number table method,with 100 cases in each group.The control group received emotional counseling,and the observation group was combined with kindred-like nursing model on the basis of nursing in the control group.After 4 weeks of nursing,the anxiety and depression status and treatment compliance before and after nursing and self-management ability after nursing were observed in the two groups.Results After nursing,the scores of Hamilton Depression Scale(HAMD and Hamilton Anxiety Scale(HAMA)of(21.93±2.62)points and(20.17±3.06)points in observation group were lower than those in control group(P<0.05).The scores of dietary compliance,compliance of drinking water,medication compliance and dialysis regimen compliance of(27.82±4.92)points,(19.74±3.91)points,(21.13±3.15)points and(17.36±2.45)points in observation group were higher than those in control group(P<0.05).The scores of partnership,problem solving,executive self-care and emotional processing were(15.22±3.16)points,(17.18±2.41)points,(21.71±2.84)points and(11.45±2.41)points in observation group,which were better than those in control group(P<0.05).Conclusion The application of emotional counseling combined with kindred-like nursing model in elderly patients with hemodialysis can improve the negative emotions and enhance the treatment compliance.
