肾盂离断成形术后差异性引流方式对肾积水患儿肾动脉血流变、BUN、Scr水平的影响比较

摘　　要：目的:研究比较肾盂离断成形术后差异性引流方式对肾积水患儿肾动脉血流变、尿素氮(BUN)、血肌酐(Scr)水平的影响。方法:选取2019年1月1日-2021年1月2日于沈阳市儿童医院接受肾盂离断成形术治疗的120例肾积水患儿,以电脑随机数字表法将其分为双J管组及硅胶外支架组,每组60例。硅胶外支架组予以硅胶外支架引流,双J管组则实施双J管引流。比较两组手术指标、肾动脉血流变指标[肾动脉阻力指数(RI)、血流收缩期峰值流速(PS)、舒张期峰值速度(ED)]、尿素氮(BUN)和血肌酐(Scr)水平及术后并发症发生情况。结果:双J管组术后住院康复时长短于硅胶外支架组,而拔管时间长于硅胶外支架组,差异均有统计学意义(P<0.05)。术前1 d和术后6个月,两组RI、PS、ED水平比较,差异均无统计学意义(P>0.05)。术后6 d,两组BUN及Scr水平均低于术前1 d,差异均有统计学意义(P<0.05)。术前1 d和术后6 d,两组BUN、Scr水平比较,差异均无统计学意义(P>0.05)。双J管组并发症发生率低于硅胶外支架组,差异有统计学意义(P<0.05)。结论:肾盂离断成形术后硅胶外支架引流以及双J管引流应用于肾积水患儿的效果相当,但双J管引流术后住院康复时长较短,且并发症发生风险较低。Objective: To study influence comparison of different drainage methods on renal artery hemorheology, blood urea nitrogen(BUN) and blood creatinine(Scr) levels in children with hydronephrosis after pyeloplasty. Method: A total of 120 children with hydronephrosis who received pyeloplasty in Shenyang Children’s Hospital from January 1, 2019 to January 2, 2021 were selected, and they were divided into double J tube group and silicone external stent group according to computer random number table method, 60 cases in each group. The silicone external stent group used silicone external stent drainage, while the double J tube group used double J tube drainage. The operative indexes, renal artery hemorheology indexes [renal arterial resistance index(RI), peak systolic flow rate(PS), peak diastolic velocity(ED)], urea nitrogen(BUN) and serum creatinine(Scr) levels and the incidence of postoperative complications were compared between two groups. Result: The length of postoperative hospitalization and recovery in the double J tube group was shorter than that in the silicone external stent group, while the extubation time in the double J tube group was longer than that in the silicone external stent group, the differences were statistically significant(P<0.05). There were no significant differences in RI, PS and ED levels between two groups 1 d before operation and 6 months after operation(P>0.05). 6 d before operation, the levels of BUN and Scr in both groups were lower than those on 1 d before operation, the differences were statistically significant(P<0.05). There were no significant differences in BUN and Scr levels between two groups on 1 d before operation and 6 d after operation(P>0.05). The complication rate in the double J tube group was lower than that in the silicone external stent group, and the difference was statistically significant(P<0.05).
