摘 要:目的:探究毛细支气管炎患儿血清中白三烯B4(LTB4)、CC趋化因子配体11(CCL11)的表达及临床意义。方法:选择2020年1月-2021年5月辽宁省健康产业集团阜新矿总医院诊治的毛细支气管炎患儿100例作为观察组,根据病情严重程度分为轻症组69例和重症组31例,同时选择同期在本院进行健康体检的100名健康儿童作为对照组。采用酶联免疫吸附法检测血清LTB4、CCL11表达水平,采用受试者工作特征(ROC)曲线评估血清LTB4、CCL11对重症毛细支气管炎的预测价值。结果:重症组血清LTB4、CCL11表达水平均高于轻症组和对照组,且轻症组均高于对照组(P<0.05)。LTB4的曲线下面积(AUC)为0.711,最佳截断值为23.45 mg/L,灵敏度、特异度分别为0.62、0.65;CCL11的AUC为0.763,最佳截断值为35.71μg/mL,灵敏度、特异度分别为0.66、0.72;LTB4联合CCL11预测毛细支气管炎严重程度的AUC为0.835,灵敏度、特异度分别为0.79、0.74。结论:血清LTB4、CCL11与毛细支气管炎严重程度密切相关,早期检测可作为预测毛细支气管炎严重程度的重要标志物。Objective: To investigate the expression and clinical significance of serum leukotriene B4(LTB4) and CC Chemokine ligand 11(CCL11) in children with bronchiolitis. Method: A total of 100 cases of children with bronchiolitis treated in Fuxin Mine General Hospital of Liaoning Health Industry Group from January 2020 to May 2021 were selected as the observation group, and according to the severity of the disease, they were divided into the mild group(n=69) and the severe group(n=31). Meanwhile, 100 healthy children who underwent physical examination in our hospital during the same period were selected as the control group. Enzyme-linked immunosorbent assay was used to detect the expression levels of serum LTB4 and CCL11, and receiver operating characteristic(ROC) curve was used to evaluate the predictive value of serum LTB4 and CCL11 on severe bronchiolitis. Result: The expression levels of LTB4 and CCL11 in severe group were higher than those in mild group and control group, and those in the mild group were higher than those in the control group(P<0.05). The area under curve(AUC) of LTB4 was 0.711, the optimal cut-off value was 23.45 mg/L, the sensitivity and specificity were 0.62 and 0.65. The AUC of CCL11 was 0.763, the optimal cut-off value was 35.71 μg/mL, the sensitivity and specificity were 0.66 and 0.72. The AUC of LTB4 combined with CCL11 was 0.835, the sensitivity and specificity were 0.79 and 0.74, respectively. Conclusion: Serum LTB4 and CCL11 are closely related to the severity of bronchiolitis, and early detection can be used as an important marker to predict the severity of bronchiolitis.
