从治风先治血试述血府逐瘀汤在颤病治疗中的临床应用

摘　　要：“治风先治血,血行风自灭”理论最早由南宋陈自明提出,后世医者在此基础上拓展发挥为从血治疗外风和内风相关疾病并获得一定疗效。作者在临证时常使用血府逐瘀汤从“治血”的角度治疗颤病这一“风证”而屡屡获效,总结分析后认为:颤病之形成,多因肝之气血阴阳失调、肝气疏泄失常而致经脉血瘀,经脉血瘀再逢肝气过盛,肝气受阻不能畅行,遂鼓动于筋脉肌肉中,使筋脉“约束不住而莫能任持”,而生颤病;治疗当用活血之法疏通气行之路,即“治风当治气,治气须行血,血行载气去,气散则风灭”。On the basis of the theory of wind should be treated by regulating blood disorder,promoting circulation of blood to extinguish the wind,which was first proposed by Chen Ziming in the Southern Song Dynasty,later doctors expanded to treat external wind and internal wind related diseases and obtain certain curative effects.The authors often use Xuefu Zhuyu decoction to treat tremor and obtain a good effect.The authors believe that the formation of tremor is mostly due to the imbalance of liver qi and blood yin and yang,liver qi drainage disorders and meridian blood stasis,meridian blood stasis and then when the liver qi is excessive,liver qi is blocked and cannot go freely,so it is agitated in the tendon muscles,so that the tendons and veins“cannot be restrained and cannot be held”,and finally become trembling;treatment should be used to dredge the path of qi,that is,“wind should be treated by regulating blood disorder,activate blood to cure qi,blood drives qi to run together to extinguish the wind.”
