小切口技术在股骨近端防旋髓内钉治疗股骨转子间骨折中的应用

摘　　要：目的探讨小切口技术辅助股骨近端防旋髓内钉(PFNA)治疗股骨转子间骨折的疗效。方法将60例股骨转子间骨折患者采用小切口技术辅助PFNA治疗。术中体外用3枚克氏针透视定位大转子顶点、定位股骨干侧方走行及股骨距位置,并按体外标记线方向置钉。术后记录骨折复位情况、骨折愈合情况。采用Harris评分评价髋关节功能。结果患者均获得随访,时间6~12个月。24例解剖复位,36例功能复位。骨折均愈合,时间3~6个月。末次随访时,53例可完全负重行走,步态无跛行,7例需扶手杖负重行走,步态轻度跛行;采用Harris评分评价髋关节功能:优46例,良10例,可4例,优良率93.3%。结论采用小切口技术辅助PFNA治疗股骨转子间骨折临床疗效满意。Objective To investigate the effect of small incision assisted proximal femoral nail antirotation(PFNA)foRtreatment of femoral intertrochanteric fracuture.Methods The 60 patients with intertrochanteric fracture of femuRwere treated with PFNA assisted by small incision technique.During the operation,three KirschneRwires were used foRfluoroscopic positioning of the vertex of the greateRtrochanter,lateral alignment of the femoral shaft and the position of the talus femur,and nails were placed in the direction of extracorporeal markeRlines.Postoperative fracture reduction and fracture healing situation were recorded,and the function of hip joint was evaluated by Harris score.Results All patients were followed up foR6~12 months.There were 24 cases of anatomic reduction and 36 cases of functional reduction.All fractures healed in 3~6 months.At the last follow-up,53 cases could walk with full weight-bearing without claudication,and 7 cases were able to walk with weight-loading undeRthe walking stick help and slight claudication.Harris evaluation of hip function:excellent 46 cases,good 10 cases,faiR4 cases,the excellent-good rate was 93.3%.Conclusions PFNA assisted with small incision technique has satisfactory clinical effect in the treatment of femoral intertrochanteric fracture.
