体外过敏原阳性儿童1646例特异性IgE分析

摘　　要：目的:分析儿童过敏患者体外过敏原特异性免疫球蛋白E(specific immune globulin E,sIgE)的分布特点。方法:1646例体外过敏原阳性患儿根据年龄分成婴儿组、幼儿组、学龄前组和学龄组,采用免疫印迹法测定19种常见体外过敏原sIgE。结果:南通地区过敏儿童的体外过敏原中,尘螨为最主要的吸入性过敏原,鸡蛋白、牛奶、牛肉为最主要的食入性过敏原。2种及以上过敏原物质过敏比例高于单一物质过敏。不同年龄段患儿对尘螨+鸡蛋白、尘螨、鸡蛋白、吸入性过敏原、食入性过敏原差异有统计学意义(P<0.01)。体外过敏原sIgE阳性浓度高低水平各不同,霉菌、树组合等12种体外过敏原sIgE阳性浓度等级主要集中在1~3级,尘螨sIgE高浓度等级分布显著高于其他过敏原。结论:不同年龄段患儿血清体外过敏原sIgE检测有利于明确体外过敏原,从而为临床提供有效的治疗依据。Objective:To analyze the distribution characteristics of allergen-positive specific globulin E(sIgE)in vitro for children.Methods:1646 children with positive allergens were divided into infant group,toddler group,preschool group,and school age group according to their ages.The Euroimmun western blotting method was used to detected the sIgE in vitro of 19 common allergens.Results:Children with allergen-positive patients in Nantong area,dust mite was the main inhaled allergen,and egg white,milk,and beef were the main ingested allergens.This study showed that the proportion of two or more allergens was higher than that of single-allergen,there was significantly difference among of the four different ages groups of children,such as dust mite mixed egg white,dust mite,egg white,inhaled and ingested allergens(P<0.001).The sIgE in vitro positive concentration levels of each allergen were different.The sIgE in vitro positive concentration levels for 12 kinds of allergens,such as molds and tree combinations,were mainly concentrated in 1-3 levels.The high concentration levels of dust mite sIgE in vitro were significantly higher than other allergens.Conclusion:The sIgE in vitro detection for different ages serum with allergen was helpful to identify allergens,and provide effective treatment prevention for clinic.
