内关穴刺激联合昂丹司琼预防剖宫产术后镇痛所致恶心呕吐的效果研究

摘　　要：目的:分析内关穴刺激联合昂丹司琼对腰麻剖宫产术后镇痛所致恶心呕吐的预防效果。方法:将50例接受腰麻剖宫产术的产妇按掷硬币法分为对照组(25例)和观察组(25例)。在术后,对两组产妇均进行静脉自控镇痛。为对照组产妇采用昂丹司琼预防恶心呕吐,为观察组产妇采用内关穴刺激法联合昂丹司琼预防恶心呕吐,然后对比两组产妇恶心呕吐的发生率等指标。结果:1)在术后,观察组产妇恶心呕吐的发生率及其中追加使用止吐药物产妇的占比均低于对照组产妇,P<0.05。观察组发生恶心呕吐的产妇中恶心呕吐分级为1级产妇的占比高于对照组发生恶心呕吐的产妇,P<0.05。2)观察组产妇术后排气的时间、术后肠鸣音恢复正常的时间及术后进食的时间均短于对照组产妇,P<0.05。在术后6 h、术后12 h、术后24 h,观察组产妇的VAS评分均低于对照组产妇,P<0.05。结论:用内关穴刺激法联合昂丹司琼预防剖宫产术后镇痛所致恶心呕吐的效果较好,能够显著缓解产妇的不适症状,促进其胃肠功能的恢复。Objective:To analyze the prevention effect of Neiguan acupoint stimulation combined with Ondansetron on nausea and vomiting caused by analgesia after cesarean section with lumbar anesthesia.Methods:fifty women who underwent cesarean section with lumbar anesthesia were divided into control group(25 cases)and observation group(25 cases)by coin toss.After the operation,the two groups of parturients were treated with intravenous controlled analgesia.The control group was given ondansetron to prevent nausea and vomiting,and the observation group was given Neiguan acupoint stimulation combined with ondansetron to prevent nausea and vomiting,and then the incidence of nausea and vomiting in the two groups were compared.Results:1)After surgery,the incidence of maternal nausea and vomiting in the observation group and the proportion of additional use of antiemetic drugs were lower than those in the control group,P<0.05.The proportion of parturients with grade 1 nausea and vomiting in the observation group was higher than that in the control group,P<0.05.2)The time of exhaust after childbirth and obstetrics,the time of bowel sounds returning to normal after operation and the time of eating after operation in the observation group were shorter than those in the control group,P<0.05.VAS score of the observation group was lower than that of the control group at 6 h,12 h and 24 h postoperatively,P<0.05.Conclusion:Neiguan acupoint stimulation combined with ondansetron has a good effect on preventing nausea and vomiting caused by postoperative analgesia after cesarean section,which can significantly relieve the discomfort symptoms of puerpera a nd promote the recovery of gastrointestinal function.
