摘 要:目的:探究肺部超声在儿童社区获得性肺炎动态监测中的价值。方法:选取2018年6月—2019年10月我院收治的疑似社区获得性肺炎儿童76例,进行肺部超声和胸部X线检查,对检查结果中胸膜线异常、胸膜下实变、胸腔积液、肺泡间质综合征等进行统计,统计肺部超声与胸部X线诊断的敏感度、特异度和准确性。结果:使用肺部超声对疑似社区获得性肺炎儿童进行检查,发现阳性72例,阴性4例。使用胸部X线对疑似社区获得性肺炎进行检查,发现阳性63例,阴性13例(P<0.05)。使用肺部超声检查对儿童社区获得性肺炎进行分析,胸膜线异常14例,胸膜下实变68例,胸腔积液7例,肺泡间质综合征50例。使用胸部X线检查对儿童社区获得性肺炎进行分析,胸膜线异常4例,胸膜下实变56例,胸腔积液1例,肺泡间质综合征37例。使用肺部超声检查对儿童社区获得性肺炎的检出率高于使用胸部X线检查的检出率(P<0.05)。肺部超声检查的敏感度、特异度、准确性均高于胸部X线检查(P<0.05)。结论:肺部超声检查对儿童社区获得性肺炎进行监测,能够准确诊断儿童肺炎,可作为胸部X线诊断的补充。Objective:To explore the value of pulmonary ultrasound in the dynamic monitoring of community-acquired pneumonia in children.Methods:Seventy-six cases of suspected community-acquired pneumonia children in our hospital from June 2018 to October 2019,lung ultrasound and chest X-ray were conducted to check the results of abnormal pleural line,subpleural change,pleural effusion,alveolar interstitial syndrome,and the diagnostic sensitivity,specificity and accuracy of lung ultrasound and chest.Results:Examination of suspected acquired pneumonia using pulmonary ultrasound found 72 positive and four negative.Examination of suspected acquired pneumonia using the chest X-ray found 63 positive and 13 negative(P<0.05).Analysis of community-acquired pneumonia in children was performed using pulmonary ultrasound with 14 abnormal pleural abnormalities,68 with subpleural changes,7 with pleural effusion and 50 with alveolar interstitial syndrome.Childhood community-acquired pneumonia was analyzed using chest X-ray,with four abnormal pleural abnormalities,56 subpleural changes,one pleural effusion and 37 alveolar interstitial syndrome.Lung ultrasound was obtained for the children’s community.The detection rate of sexual pneumonia was higher than that for community-acquired pneumonia in children using chest X-ray(P<0.05).The sensitivity,specificity and accuracy of lung ultrasound were higher than the sensitivity,specificity and accuracy of chest X examination(P<0.05).Conclusion:Lung ultrasound monitors community-acquired pneumonia in children,enables an accurate diagnosis of childhood pneumonia,and can serve as a diagnostic supplement to the chest X-ray.
