摘 要:脾虚膏脂转输障碍是动脉粥样硬化性心血管疾病核心病机。肠道菌群作为人体"第二基因组",在肠道脂质吸收、输送以及利用等过程中发挥至关重要作用。肠道菌群稳态失衡是脾虚膏脂转输障碍的关键环节。以肠道菌群驱动脂质代谢为切入点,深入揭示动脉粥样硬化性心血管疾病发病机制以提高中西医结合防治水平逐渐成为基础和临床研究焦点和热点。从肠道菌群靶向磷脂酰胆碱/ApoA-Ⅰ/HDL亚类代谢的角度,深入分析脾失健运-肠道菌群失调-膏脂转输障碍的分子生物学基础,为筛选以肠道菌群驱动磷脂酰胆碱/ApoA-Ⅰ/HDL亚类代谢为有效靶点的健脾降脂中药及中西医结合防治动脉粥样硬化性心脑血管疾病提供新的研究策略及科学依据。Spleen deficiency leading to the dysfunction of grease transfer is the core pathogenesis of atherosclerotic cardiovascular disease.As the"second genome"of human body,intestinal flora plays an important role in the absorption,transportation and utilization of intestinal lipids.The steady-state imbalance of intestinal flora is the key link of the disturbance of lipid transfer in spleen deficiency ointment.From the point of view of lipid metabolism driven by intestinal flora,it has gradually become the focus of basic and clinical research to reveal the pathogenesis of atherosclerotic cardiovascular disease in order to improve the level of prevention and treatment of integrated traditional Chinese and western medicine.From the point of view of phosphatidylcholine/ApoA-Ⅰ/HDL subclass metabolism targeted by intestinal flora,this paper deeply analyzed the molecular biological basis of dysfunction of spleen in transportation,intestinal flora imbalance and dysfunction of grease transfer,so as to provide a new research strategy and scientific basis for screening spleen-invigorating and lipid-lowering herbs with intestinal flora-driven phosphatidylcholine/ApoA-Ⅰ/HDL subclass metabolism as an effective target and integrated traditional Chinese and Western medicine for the prevention and treatment of atherosclerotic cardio-cerebrovascular diseases.
