健脾益肾方对糖尿病营养状况及免疫功能的影响分析

摘　　要：目的探究健脾益肾方对2型糖尿病患者营养状况及免疫功能的影响。方法随机将医院2017年1月—2020年1月诊治90例2型糖尿病患者分为对照组和治疗组,两组各45例,对照组给予二甲双胍治疗,治疗组在对照组的基础上给予健脾益肾方,两组疗程均为12周。观察两组患者治疗前后血糖指标水平、营养状况、免疫功能及不良反应发生情况。结果治疗后,两组患者空腹血糖(FPG)、糖化血红蛋白(HbA1c)、餐后2 h血糖(2 h PG)均显著下降(P<0.05),且治疗组低于对照组(P<0.05);治疗后,两组患者上臂肌围(AMC)、体质指数(BMI)均较治疗前升高,治疗组高于对照组,差异无统计学意义(P>0.05),改良主观整体评估法评分系统(MOSGA)和营养不良炎症评分(MIS)显著降低(P<0.05),治疗组低于对照组(P<0.05);治疗后两组T淋巴细胞亚群CD_(4)^(+)及CD_(4)^(+)/CD_(8)^(+)水平显著高于治疗前(P<0.05),免疫球蛋白IgA、IgG水平显著降低(P<0.05),IgM水平显著升高(P<0.05),治疗组均优于对照组(P<0.05);两组患者均无明显不良反应,且差异无统计学意义(P>0.05)。结论健脾益肾方治疗糖尿病效果显著,可降低患者血糖指标,改善患者营养状况,提高患者免疫功能,无严重不良反应,可临床推广应用。Objective To investigate the effect of Jianpi Yishen Recipe(健脾益肾方)on nutritional status and immune function of diabetic patients.Methods Ninety patients with diabetes diagnosed and treated in our hospital from January 2017 to January 2020 were randomly divided into a control group and a treatment group,with 45 cases in each group.The control group was treated with metformin,and the treatment group was given Jianpi Yishen Recipe on the basis of the control group.The course of treatment in both groups were 12 weeks.The blood glucose level,nutritional status,immune function and adverse reactions in both groups were observed before and after treatment.Results After treatment,the fasting blood glucose(FPG),glycated hemoglobin(HbA1 c)and two-hour postprandial blood glucose(2 h PG)of the two groups were significantly decreased(P<0.05),and those of the treatment group were lower than those of the control group(P<0.05).After treatment,the upper arm muscle circumference(AMC)and body mass index(BMI)of the two groups were higher than those before treatment,and those of the treatment group were higher than those of the control group.The difference was not statistically significant(P>0.05),and the improved subjective overall assessment scoring system(MOSGA)and malnutrition and inflammation score(MIS)were significantly reduced(P<0.05).Those of the treatment group were significantly lower than those of the control group(P<0.05).After treatment,the levels of T lymphocyte subsets CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)were significantly higher than those before treatment(P<0.05).The levels of immunoglobulin IgA and IgG were significantly reduced(P<0.05),and the level of IgM was significantly increased(P<0.05).The treatment group were better than the control group(P<0.05).None of the patients had obvious adverse reactions,and the difference was not statistically significant(P>0.05).Conclusion Jianpi Yishen Recipe has a significant effect on the treatment of diabetes.It can lower the blood sugar index,improve the nutritional s
