调脂清脑汤结合苯磺酸氨氯地平片治疗原发性高血压病的临床疗效及对血液流变学的影响

摘　　要：目的评价调脂清脑汤结合苯磺酸氨氯地平片治疗原发性高血压病的临床疗效及对血液流变学的影响。方法以2020年1月—2021年1月医院收治的原发性高血压病患者75例为研究对象,依据治疗方法的不同分为联用组(39例)和西药组(36例)。西药组患者口服苯磺酸氨氯地平片治疗,1片/d,1次/d,晨起口服。联用组患者采用调脂清脑汤结合苯磺酸氨氯地平片治疗。两组分别治疗1个月。比较两组治疗前后眩晕、头痛、暴躁易怒、心悸和失眠等证候积分变化,比较两组治疗前后收缩压(SBP)、舒张压(DBP)水平变化,比较两组治疗前后血液流变学指标(全血高切黏度、全血低切黏度、血浆比黏度和红细胞聚集指数)水平,比较治疗疗效。结果联用组治疗总有效率高于西药组(P<0.05);两组治疗后眩晕、头痛、暴躁易怒、心悸和失眠等证候积分较治疗前降低,且联用组降低幅度优于西药组(P<0.05);两组治疗后SBP、DBP和脉压差较治疗前降低,且联用组降低幅度优于西药组(P<0.05);两组治疗后全血高切黏度、全血低切黏度、血浆比黏度和红细胞聚集指数较治疗前降低,且联用组降低幅度优于西药组(P<0.05)。结论调脂清脑汤结合苯磺酸氨氯地平片治疗原发性高血压病疗效确切,可显著改善原发性高血压病患者的中医证候积分,控制血压水平,降低血压黏稠度,改善血液流变学指标,提高治疗疗效,值得临床进一步研究探讨。Objective To evaluate the clinical efficacy of Tiaozhi Qingnao Decoction(调脂清脑汤)combined with Amlodipine Sulfonate Tablet in the treatment of essential hypertension and its effect on hemorheology.Methods Seventy-five patients with essential hypertension admitted to our hospital from January 2020 to January 2021 were selected as the research objects.According to the different treatment methods,they were divided into combined treatment group(39 cases)and western medicine group(36 cases).Western medicine group was treated with Amlodipine Besylate Tablet orally,one tablet a day,once a day,taking in the morning.The combined treatment group was treated with Tiaozhi Qingnao Decoction combined with Amlodipine Benzenesulfonate Tablet.The two groups were treated for 1 month.The changes of symptom scores such as vertigo,headache,irritability,palpitation and insomnia before and after treatment,the changes of systolic blood pressure(SBP)and diastolic blood pressure(DBP)before and after treatment and the levels of hemorheological indexes(whole blood high shear viscosity,whole blood low shear viscosity,plasma specific viscosity and erythrocyte aggregation index)before and after treatment were compared.Results The total effective rate of the combined treatment group was higher than that of the western medicine group(P<0.05).After treatment,the scores of symptoms such as vertigo,headache,irritability,palpitation and insomnia in the two groups were lower than those before treatment,and the reduction rate in the combined treatment group was better than that in the western medicine group(P<0.05).After treatment,SBP,DBP and pulse pressure in the two groups were lower than those before treatment,and the reduction rate in the combined treatment group was better than that in the western medicine group(P<0.05).After treatment,the whole blood high-cut viscosity,whole blood low-cut viscosity,plasma specific viscosity and erythrocyte aggregation index of the two groups were lower than those before treatment,and the reduction range
