盐酸罗哌卡因联合氢吗啡酮腰椎麻醉在剖宫产术中的应用效果

摘　　要：目的:探讨盐酸罗哌卡因联合氢吗啡酮腰椎麻醉在剖宫产术中的应用效果。方法:选取90例剖宫产产妇为研究对象,按照随机数字表法分为对照组和观察组各45例。对照组采用盐酸罗哌卡因腰椎麻醉,观察组采用盐酸罗哌卡因联合氢吗啡酮腰椎麻醉,比较两组麻醉优良率、疼痛[视觉模拟评分法(VAS)]评分、应激反应指标[C反应蛋白(CRP)、葡萄糖(GLU)、皮质醇(COR)]水平和不良反应发生率。结果:观察组产妇麻醉优良率明显高于对照组,差异有统计学意义(P<0.05);两组产妇术后2 h VAS评分比较,差异无统计学意义(P>0.05),术后6 h、12 h、24 h观察组VAS评分均明显低于对照组,差异有统计学意义(P<0.05);麻醉后、术后1 h,两组CRP、GLU、COR水平均高于麻醉前,但观察组低于对照组,差异有统计学意义(P<0.05)。两组产妇不良反应发生率比较,差异无统计学意义(P>0.05)。结论:盐酸罗哌卡因复合氢吗啡酮腰椎麻醉应用于剖宫产术可提高产妇麻醉优良率,降低VAS评分和应激反应指标水平,优于单纯盐酸罗哌卡因腰椎麻醉效果。Objective:To observe application effects of Ropivacaine hydrochloride combined with Hydromorphone lumbar anesthesia in cesarean section.Methods:90 cases with cesarean section were selected as the research objects,and were divided into control group and observation group according to the random number table method,45 cases in each group.The control group received lumbar anesthesia with Ropivacaine hydrochloride,while the observation group received Ropivacaine hydrochloride combined with Hydromorphone lumbar anesthesia.The excellent and good rate of anesthesia,the pain[visual analogue scale(VAS)]score,the stress response index[C-reactive protein(CRP),glucose(GLU),cortisol(COR)]levels and the incidence of adverse reactions were compared between the two groups.Results:The excellent and good rate of anesthesia in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the VAS score 2 hours after the surgery between the two groups at(P>0.05).The VAS scores of the observation group were significantly lower than those of the control group 6 hours,12 hours,and 24 hours after the surgery,and the differences were statistically significant(P<0.05).The levels of CRP,GLU and COR in the two groups after anesthesia and 1 hour after the surgery were higher than those before the surgery;those of the observation group were lower than those of the control group;and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Ropivacaine hydrochloride combined with Hydromorphone lumbar anesthesia applied to cesarean section can improve the excellent and good rate of anesthesia,and reduce the VAS score and the stress response index levels.Moreover,it is superior to single Ropivacaine hydrochloride lumbar anesthesia.
