摘 要:目的:观察神经肌肉电刺激联合摄食训练治疗卒中后吞咽障碍患者的效果。方法:选取81例卒中后吞咽障碍患者为研究对象,按照随机数字表法分为对照组(n=40)和观察组(n=41)。对照组采用常规摄食训练治疗,观察组在对照组的基础上联合神经肌肉电刺激治疗,比较两组临床疗效、干预前后舌骨喉复合体动度、营养指标[肱三头肌皮褶厚度(TSF)、健侧上臂中部肌围(AMC)、体质量指数(BMI)]水平及并发症发生率。结果:观察组治疗总有效率为95.12%,高于对照组的77.50%,差异有统计学意义(P<0.05);观察组舌骨前移、舌骨上移等舌骨喉复合体动度均大于对照组,差异有统计学意义(P<0.05);观察组TSF、AMC、BMI等营养指标水平均高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为14.64%,低于对照组的27.50%,差异有统计学意义(P<0.05)。结论:神经肌肉电刺激联合摄食训练治疗卒中后吞咽障碍患者,可提高临床疗效和舌骨喉复合体动度,改善患者营养水平,降低并发症发生率,其效果优于单纯常规摄食训练。Objective:To observe effects of neuromuscular electrical stimulation combined with feeding training in patients with dysphagia after stroke.Methods:81 patients with dysphagia after stroke were selected as the research objects,and were divided into control group(n=40)and observation group(n=41)according to the random number table method.The control group was given routine feeding training,while the observation group was given neuromuscular electrical stimulation on the basis of that of the control group.The clinical effects,the hyoid-laryngeal complex motility level,the nutritional index levels[triceps skinfold thickness(TSF),unaffected side mid-upper arm muscle circumference(AMC),body mass index(BMI)]and the incidence of complications were compared between the two groups before and after the intervention.Results:After the intervention,the total effective rate of treatment in the observation group was 95.12%,which was higher than the control group of 77.50%,and the difference was statistically significant(P<0.05).The hyoid-laryngeal complex motility levels in the observation group such as hyoid bone anterior and superior moving were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of TSF,AMC,BMI and other nutritional indexes in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 14.64%,which was lower than 27.50%in the control group,and the difference was statistically significant(P<0.05).Conclusions:neuromuscular electrical stimulation combined with Feeding training in the treatment of the post-stroke dysphagia patients can improve the clinical effects and the hyoid-laryngeal complex mobility levels,improve the nutritional levels of these patients,and reduce the incidence of complications.Moreover,it is superior to single routine feeding training.
