加速康复外科护理在腹腔镜全胰腺切除术患者中的应用效果

摘　　要：目的:观察加速康复外科(ERAS)护理在腹腔镜全胰腺切除术患者中的应用效果。方法:选取85例行腹腔镜全胰腺切除术患者为研究对象,按照随机数字表法分为研究组42例与对照组43例。对照组开展常规围术期护理,研究组则开展ERAS护理,比较两组临床相关指标(首次饮水时间、首次排气时间、首次下床活动时间、首次进食时间、住院时间)、住院期间并发症发生率和正性负性情绪量表(PANAS)评分。结果:研究组首次饮水时间、首次排气时间、首次下床活动时间、首次进食时间、住院时间均短于对照组,差异有统计学意义(P<0.05);住院期间,两组胰瘘、感染、术后出血等并发症发生率比较,差异均无统计学意义(P>0.05);出院时,两组PA评分均高于护理前,且研究组高于对照组,两组NA评分均低于护理前,且研究组低于对照组,差异有统计学意义(P<0.05)。结论:ERAS护理用于腹腔镜全胰腺切除术患者可缩短其首次饮水时间、首次排气时间、首次下床活动时间、首次进食时间和住院时间,并改善其PANAS评分,效果优于常规护理。Objective:To observe application effects of enhanced recovery after surgery(ERAS)nursing in patients with laparoscopic total pancreatectomy.Methods:85 patients undergoing laparoscopic total pancreatectomy were selected as the research subjects,and were divided into study group(n=42)and control group(n=43)according to the random number table method.The control group was given routine perioperative nursing,while the study group was given the ERAS nursing.The clinically relevant indicators(first drinking time,first exhausting time,first getting out of bed time,first eating time,hospitalization time),the incidence of complications during the hospitalization and the positive and negative affect scale(PANAS)score were compared between the two groups.Results:The first drinking time,the first exhausting time,the first getting out of bed,the first eating time,and the hospitalization time in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).During the hospitalization,there were no significant differences in the incidence of pancreatic fistula,infection,postoperative bleeding and other complications between the two groups(P>0.05).At discharge,the PA scores of the two groups were higher than those before the nursing,and that of the study group was higher than that of the control group;the NA scores of both groups were lower than those before the nursing,and that of the study group was lower than the control group;and the differences were statistically significant(P<0.05).Conclusions:ERAS nursing for the patients undergoing laparoscopic total pancreatectomy can shorten their first drinking time,first exhausting time,first getting out of bed time,first eating time,and hospitalization time,and improve their PANAS scores.Moreover,it is superior to the routine nursing.
