血糖与糖化血红蛋白联合检测对提高社区老年糖尿病筛选准确性的研究

摘　　要：目的:通过与传统方法的比较,评价血糖和糖化血红蛋白联合检测在社区糖尿病筛查诊断中的运用价值。方法:纳入2020年1月至2021年12月无锡市河埒街道社区卫生服务中心收治的70例糖尿病和其他非糖尿病患者,同时采用尿常规检测、口服葡萄糖耐量试验(OGTT)及血糖与糖化血红蛋白联合方案对所有临床样本进行检测,并对结果进行分析比较。结果:联合检测方案的筛查准确性为97.5%,略低于OGTT 100%的准确性,差异无统计学意义(P>0.05);尿常规检测的准确性为67.5%,与联合检测方案相比,差异有统计学意义(P<0.05)。联合检测方案的检测时间与尿常规检测相当,都为30 min,较OGTT的3h要快,差异有统计学意义(P<0.01)。结论:血糖与糖化血红蛋白联合检测方案不仅结果准确可靠,且操作简便更适用于在社区机构老年群体糖尿病筛查中的开展使用,为我国糖尿病群体的早诊断、早控制提供了另一个有效的途径。Objective:By comparing with traditional methods,we evaluated the value of combined detection of blood glucose and glycosylated hemoglobin in community diabetes screening and diagnosis.Methods:A total of 70 clinical samples were collected from diabetes and other non-diabetic population admitted to the Wuxi River Street Community Health Center in January 2020 to December 2021,At the same time,all clinical samples were detected by routine urine test,Oral Glucose Tolerance Test(OGTT),blood glucose and glycosylated hemoglobin,and the results were analyzed and compared.Results:The screening accuracy of the combined detection scheme was 97.5%,slightly lower than OGTT’s 100%,the difference was not statistically significant(P>0.05).The accuracy of routine urine test was 67.5%,compared with the combined detection scheme,the difference was statistically significant(P<0.05).The detection time of the combined detection scheme was equivalent to routine urine detection,which was faster than OGTT’s three hours,the difference was statistically significant(P<0.01).Conclusion:The combined detection scheme of blood glucose and glycosylated hemoglobin is not only accu rate,reliable,and easy to operate,but also applicable to the screening of diabetes in the elderly population in community institutions.It provides another effective way for early diagnosis and early control of diabetic groups in China.
