抗核抗体检测和抗核抗体谱检测在复发性流产诊断中的应用分析

摘　　要：目的:通过对复发性流产(RSA)患者抗核抗体(ANA)检测结果的分析,探讨ANA检测与RSA的相关性。方法:采用间接免疫荧光法(IIF)测定2020年7~12月收集的1432例RSA患者(观察组)与同时期无不良孕产史的134例正常孕妇(对照组)的ANA,采用免疫印迹法(LIA)测定其ANA谱。比较两个组别检测结果,并分析其与RSA关系。结果:观察组中检出ANA阳性292例(20.39%)。对照组检出ANA阳性9例(6.72%)。观察组常见ANA核型为颗粒型(36.32%)和致密型(32.89%)。观察组ANA谱检测,各类抗体均有检出,其中抗Ro-52抗体(24.55%)和抗SS-A抗体(24.19%)检出率最高,ANA阴性者也有ANA谱检出。ANA与ANA谱联合检测,两种方法联合检测阳性率达到27.65%,两种方法联合检测总体符合率达到.79.40%,观察组ANA和ANA谱联合检测的灵敏度为98.49%,特异度为99.61%;观察组联合检测阳性率显著高于对照组,差异有统计学意义(χ^(2)=20.47,P<0.01)。结论:ANA及ANA谱联合检测有助于RSA的临床诊断。Objective:To investigate the correlation between ANTinuclear antibody(ANA)and RSA in patients with recurrent abortion(RSA).Methods:Indirect immunofluorescence assay(IIF)was used to measure ANA in 1432 RSA patients(observation group)collected from July to December 2020 and 134 normal pregnant women(control group)without adverse pregnancy history during the same period.The results of the two groups were compared,and the relationship between RSA and the two groups was analyzed.Results:In the observation group,292 cases(20.39%)were ANA positive.In the control group,9 cases(6.72%)were ANA positive.The common ANA karyotypes in the observation group were granular(36.32%)and dense(32.89%).ANA spectrum detection showed that all kinds of antibodies were detected in the observation group,among which anti-Ro-52 antibody(24.55%)and anti-SS-A antibody(24.19%)had the highest detection rates.ANA spectrum detection was also found in ANA negative patients.ANA and ANA spectrum combined detection,the positive rate of the combined detection of the two methods reached 27.65%,the overall coincidence rate of the combined detection of the two methods reached 79.40%,the sensitivity and specificity of the combined detection of antinuclear antibody and antinuclear antibody spectrum of the observation group was 98.49%,99.61%.The positive rate of combined de tection in observation group was significantly higher than that in control group,and the difference was statistically significant(χ^(2)=20.47,P<0.01).Conclusion:The combined detection of ANA and ANA spectrum is helpful for the clinical diagnosis of RSA.
