不同腹腔镜术式治疗妇科疾病的效果分析

摘　　要：目的:对比改良Port阴道单孔腹腔镜与经腹部多孔腹腔镜手术治疗妇科疾病的效果。方法:选取2019年4月至2020年10月收治的92例需行腹腔镜手术的妇科疾病患者,采用随机数字表法将患者分为两组,每组46例,单孔组行改良Port阴道单孔腹腔镜手术,多孔组行经腹部多孔腹腔镜手术。对比分析两组手术创伤性相关指标(手术时间、术中出血量)、术中麻醉用药情况、术后镇痛泵按压次数、肛门首次排气时间、手术医用耗材费用、住院时间、患者满意度及术后并发症、切口愈合情况。结果:两组手术时间、术中出血量、麻醉药物用量差异无统计学意义(P>0.05);单孔组术后肛门首次排气时间、住院时间短于多孔组(P<0.05),术后镇痛泵按压次数、手术医用耗材费用少于多孔组(P<0.05);两组满意度等级分布情况差异有统计学意义(P<0.05),单孔组总满意率高于多孔组(P<0.05)。术后随访6个月,两组均无并发症发生,切口愈合良好。结论:相较经腹部多孔腹腔镜手术,改良Port阴道单孔腹腔镜手术治疗妇科疾病具有手术创伤小、术后康复快的优势,且经济、安全。Objective:To compare the application effect of modified port vaginal single port laparoscopy and transabdominal multiple ports laparoscopy in the treatment of gynecological diseases.Methods:From Apr.2019 to Oct.2020,92 patients with gynecological diseases requiring laparoscopic surgery were selected and divided into two groups by random number table method,each consisted of 46 cases.Patients in single port group underwent modified port vaginal single port laparoscopic operation,while patients in multiple ports group underwent transabdominal multiple ports laparoscopic surgery.The related indexes of surgical trauma(operation time and intraoperative blood loss),intraoperative anesthetics,the pressure times of analgesia pump after surgery,the first exhaust time of anus after operation,surgical medical consumables cost,hospital stay,satisfaction of patients,postoperative complications and incision healing were compared between the two groups.Results:There was no significant difference in operation time,intraoperative blood loss and anesthesia dosage between the two groups(P>0.05).The first postoperative anal exhaust time and hospital stay in single port group were shorter than those in multiple ports group(P<0.05).The pressure times of analgesia pump and the cost of surgical medical consumables in single port group were less than those in multiple ports group(P<0.05).There was significant difference in the grade distribution of satisfaction between the two groups(P<0.05),and the total satisfaction rate of single port group was higher than that of multiple ports group(P<0.05).All the patients were followed up for 6 months after operation,there were no complications in both groups,and the incision healed well.Conclusions:Compared with transabdominal multiple ports laparoscopy,the modified port vaginal single port laparoscopic surgery has advantages of reduced trauma and accelerated recovery,and it is economical and safe.
