某三级医院国产及进口医疗器械管理与使用现状对比研究

摘　　要：目的基于疾病诊断相关分组(diagnosis related groups,DRGs)在医院绩效管理中的运用,不同种类的医疗器械应选择国产还是进口品牌,为临床提供合理化建议.方法通过参加展会,加强与医疗器械企业交流,了解目前国产与进口医疗器械发展情况;查阅相关网站、文件,掌握与医疗器械生产、管理、使用等相关政策;汇总2018~2020年某三级医院国产与进口医疗器械的数量;选取两种手术器械,介绍国产品牌的技术创新;选取电子胆道镜和移动DR,对比分析国产与进口品牌产品的经济效益.结果近几年国产医疗器械呈现由低端向中高端创新发展的趋势,逐步打破进口中高端医疗器械垄断市场的局势.该医院近三年国产医疗器械数量的增长幅度大于进口医疗器械,但主要还是以技术成熟的低端医疗器械为主.选取的两种手术器械的国产品牌已完全可以替代进口品牌,并在技术性能方面进一步创新;对比选取的电子胆道镜及移动DR两种医疗器械国产与进口品牌的效益分析,国产品牌的性价比要稍优于进口品牌.结论医院应结合当下政策导向、医疗器械技术性能、经济效益等因素综合考虑确定使用国产或进口医疗器械.Objective Based on the application of diagnosis related groups(DRGs)in hospital performance management,different kinds of medical devices should choose domestic brand or imported brand,so as to provide reasonable and scientific suggestions for clinical practice.Methods Through participating in the medical device exhibition,we strengthened the communication with medical device manufacturers to understand the current development of domestic and imported medical devices;Consult relevant websites and documents to master the policies related to the production,management and use of medical devices;The number of domestic and imported medical devices of a tertiary hospital in recent three years from 2018 to 2020 was summarized;Two kinds of surgical instruments were selected to introduce the technological innovation of domestic brands.Electronic choledochoscope and mobile DR were selected to analyze the benefits of domestic and imported brands respectively.Results The analysis shows that in recent years,domestic medical devices present the trend of innovation and development from low-end to medium and high-end,and gradually break the monopoly market situation of imported medium and high-end medical devices.In the past three years,the growth rate of domestic medical devices is greater than that of imported medical devices,but the low-end medical devices with mature technology are the main ones;The domestic brands of the two selected surgical instruments can completely replace the imported brands,and further innovate in the technical performance;Comparing the benefit analysis of domestic and imported brands of electronic choledochoscope and mobile DR,the cost performance of domestic brands is slightly better than that of imported brands.Conclusion The hospital should combine the current policy guidance,the technical performance of medical devices,economic benefits and other factors to determine whether different types of medical devices choose domestic brand or imported brand.
